降钙素原在细菌性呼吸道感染疾病中的早期诊断及临床价值

摘要 目的:探讨降钙素原(PCT )对早期细菌性呼吸道感染性疾病的临床诊断意义及预后评估价值。方法采用回顾性分析,选取2014年4‐10月于佛山市第一人民医院的治疗呼吸道感染患者117例,平均年龄为46岁,其中细菌感染患者60例,非细菌感染患者57例,比较呼吸道细菌培养结果与血清 PCT 浓度的关系,比较革兰阴性菌感染与革兰阳性菌感染之间 PCT 浓度水平的差异以及观察阴性菌感染患者治疗前后血清 PCT 浓度水平的差异。结果随着患者血清 PCT 值的升高,其痰培养阳性率越高,革兰阴性菌感染者的血清 PCT 值明显高于革兰阳性感染者(P<0.05),阴性菌感染患者治疗前后血清 PCT 浓度水平相比有统计学意义(P <0.05)。结论患者血清 PCT 的水平可以对早期细菌性呼吸道感染疾病作出辅助诊断,有助于指导早期的治疗用药及预后作出评估。 Objective To evaluate the significance of clinical diagnosis and the value of prognosis for early bac‐terial respiratory infections .Methods adopted the retrospective analysis ,117 cases of respiratory tract infections with an average age of 46 were selected in the First People′s Hospital of Foshan City in Guangdong Province from April 2014 to October 2014 ,and bacterial infections group with 60 cases ,non‐bacterial infections group with 57 cases ,the relationship between bacterial culture results of respiratory and the concentrations of serum PCT were compared .The serum PCT levels between Gram‐negative bacterial infections and negative bacterial infections were compared .The differences of Gram‐negative bacterial infections serum PCT levels were observed before and after treatment .Results With the increase of the value of serum PCT ,positive rate of sputum culture was increasing ,serum PCT values of Gram‐negative bacterial infections was significantly higher than those of Gram‐positive infections(P 〈 0 .05) .serum PCT of Gram‐negative bacterial infections was statistically significant before and after treatment (P〈 0 .05) .Conclu‐sion Serum PCT has made a diagnosis for bacterial respiratory infection in early stage and has been beneficial for guiding treatment and assess for prognosis .
