联合全血超敏C-反应蛋白和白细胞计数在儿童呼吸道感染中的应用价值 被引量:11

摘要 目的:探讨全血超敏C-反应蛋白(hypersensitive C-reactive protein,hs-CRP)联合白细胞计数(White blood cell count,WBC)在儿童呼吸道感染中的应用价值。方法:临床纳入73例呼吸道细菌性感染患儿与47例呼吸道病毒性感染患儿。此外,研究纳入50例健康的儿童作为研究对照。分别检查上述各组患儿全血hs-CRP与WBC水平,并进行分析。结果:细菌性感染组hs-CRP、WBC水平分别为(18.25±4.32)mg/L、(13.55±1.05)×109L-1,病毒性感染组hs-CRP、WBC水平分别为(4.05±2.91)mg/L、(7.97±2.11)×109L-1,健康组hs-CRP、WBC水平分别为(2.97±0.75)mg/L、(7.05±1.95)×109L-1,差异有显著性(P<0.05);治疗后,细菌性感染组hs-CRP、WBC水平较治疗前明显下降,差异有显著性(P<0.05);hs-CRP≥8mg/L时,hs-CRP诊断细菌感染灵敏度、特异度、阳性预测值、阴性预测值分别为86.30%、50.68%、56.16%、83.56%;WBC≥10×109L-1时,hs-CRP诊断细菌感染灵敏度、特异度、阳性预测值、阴性预测值分别为82.19%、68.49%、65.75%、83.56%。结论:hs-CRP与WBC对诊断及鉴别儿童呼吸道感染具有较高的指导价值,其中hs-CRP在诊断细菌性感染时具有更高的灵敏度、特异度以及准确率。 Objective: To study the application value of hs-CRP combined with WBC in children with respiratory tract infection. Method: 73 children with respiratory tract bacterial infection,47 children with viral respiratory infection and 50 healthy children were selected. The levels of hs-CRP and WBC of each group were detected and analyzed. In addition,the changes of hs-CRP and WBC of bacterial infection and viral infection group before and after treatment were observed. Regarding the positive criteria of hs-CRP ≥8mg / L and WBC≥10×10^9L^-1,the sensitivity,specificity,positive predictive value and negative predictive value of bacterial infection based on hs-CRP and WBC was calculated. Result: The levels of hs-CRP and WBC of bacterial infection group were( 18.25±4.32) mg / L and( 13.55 ± 1.05) × 10^9L^-1; the levels of hs-CRP and WBC of viral infection group were( 4.05±2.91) mg / L and( 7.97±2.11) ×10^9L^-1,while the levels of healthy group were( 2.97±0.75) mg / L and( 7.05±1.95) ×10^9L^-1( P0.05); after treatment,the levels of hs-CRP and WBC of bacterial infection group were significantly lower than before( P〈0.05); when hs-CRP≥8mg /L,the sensitivity,specificity,positive predictive value and negative predictive value of bacterial infection based on hs-CRP was 86.30%,50.68%,56.16% and 83.56%; when WBC≥10 × 109L-1,the sensitivity,specificity,positive predictive value and negative predictive value of bacterial infection based on WBC was82.19%,68.49%,65.75% and 83.56%.Conclusion: hs-CRP and WBC has a higher guiding value in the diagnosis and identification of respiratory tract infection. During the diagnosis process of bacterial infection,hs-CRP has a higher sensitivity,specificity and accuracy.
