卵巢颗粒细胞瘤的MRI弥散成像与动态增强表现分析 被引量:12

摘要 目的回顾性分析卵巢颗粒细胞瘤的MRI表现及弥散成像、动态曲线表现。方法收集10例经病理证实的卵巢颗粒细胞瘤患者临床及MRI资料,并行DWI(0,800)扫描及ADC值测量,其中9例行时间-信号曲线图测量并记录。结果成人型颗粒细胞瘤(OAGCT)9例;幼年型颗粒细胞瘤(OJGCT)1例。8例病灶表现为囊实混合性,T2WI呈混杂信号、以高信号为主,T1WI呈稍高及稍低信号,DWI呈高、低混杂信号;2例病灶表现为实性,T2WI及T1WI信号均略高于肌肉,DWI呈高信号。平均ADC值:实性成分:0.351-0.881×10-3mm2/s;囊性成分:1.510-3.447×10-3mm2/s。时间信号曲线表现为缓升或缓升平台7例,速升平台型2例,病灶曲线高度低于子宫肌层。结论卵巢颗粒细胞瘤以囊实混合性多见,DWI为混杂信号或高信号,实性成分ADC值低,囊性成分ADC值高,时间信号曲线以缓升或缓升平台型多见,MRI表现具有一定特点。 Objective To analyze the MR and clinicopathological characteristics of ovarian granulosa call tumor (OGCT) so as to improve the diagnostic accuracy. Methods Clinical and MRI materials of 10 OGCTs proved by pathology were collected. They underwent DWI (0,800) scanning. ADC values and time-signal intensity curves were acquired on the workstation. Results 9 cases were diagnosed as adult OGCT ( adult-type granulose cell tumor of ovary, OAGCT) , and one case was diagnosed as juvenile OGCT (juvenile-type granulose cell tumor of ovary, OJGCT) by pathology. 8 lesions presented as cystic-solid mass with mixed high-intensity signal on T2WI, slightly low and high-intensity signal on T1WI, mixed intensity signal on DWI. 2 were solid masses, with the lesions' signal being a little more higher than that of muscle on both T1WI and T2WI, and the signal of DWI was high-intensity. Mean values of ADC were 0. 351-0. 881 × 10^-3 mm^2/s ( solid area) ; 1. 510-3. 447×10^-3 mm^2/s (cystic area). Time-signal intensity curve (TIC) was that 7 cases were the type of slow-rise or slow-rise-platform, 2 cases were the fast-rise-platform,the peak value of lesion was under that of myometrium. Conclusion OGCTs were cystic-solid masses in the majority. The signals on DWI were mixed or high. The ADC values of solid area were relatively low, and that of cystic area were relatively high. The types of TIC were slow-rise or slowriseplatform at most. MRI appearances of OGCT had its own characters.
