摘要 目的:探讨放射性^(125)I粒子气道支架治疗恶性中心气道狭窄的临床疗效及安全性。方法:27例恶性中心气道狭窄患者,根据病情定制气道支架,并将放射性^(125)I粒子捆绑在气道支架表面,据肿瘤组织进行分布,经支气管镜下植入,术后24h、1月、2月、3月随访评估疗效和并发症。结果:27例患者均一次性成功植入放射性^(125)I粒子气道支架,支架植入后患者狭窄管腔较前扩张,生活质量评分增加,26例患者术后呼吸困难较前改善。随访:1例患者支架轻度移位,1例患者发生管腔再狭窄,1例患者气道内分泌物潴留轻度影响通气。结论:放射性^(125)I粒子气道支架治疗恶性中心气道狭窄安全可行、疗效肯定、并发症少,是一种有效的姑息治疗手段。 Objective:To discuss the clinical efficacy and safety of 125I radioactive particles stent in the treatment of patients with malignant central airway stenosis.Methods:27 cases with malignant central airway stenosis were selected for further study.The customized airway stents covering with 125I radioactive particles were implanted according to the distribution of the tumor tissue under the guidance of bronchoscopy.The efficacy and complications were assessed at 24 hours,1 month,2 months and 3 months after the operation.Results:27 patients were successfully implanted with 125I radioactive particle stents in one time.The narrowed bronchi were expanded and the qualities of life were increased after the implantation of the airway stent.After operation,breath shortness of 26 cases were better than before.Follow up:1 patient had mild stent displacement,1 patient had bronchial restenosis,1 patient had mild influence of ventilate due to the retention of secretions in the airway.Conclusion:Radioactive 125I particle airway stent is a safe and effective palliative treatment for patients with malignant central airway stenosis.
