摘要
目的通过动物实验观察不同套囊内压对气管黏膜的影响,探索出对气管黏膜具有保护作用的最适套囊内压力范围。方法健康家兔60只,3-3.5月龄,雌雄不限,体重3-4kg,随机分为3组,每组20只。即低套囊内压组(A组):套囊内压20mmHg;中套囊内压组(B组):套囊内压25mmHg;高套囊内压组(C组):套囊内压30mmHg。气管导管型号:ID3.5mm,生产厂家:Covidien IIc。于套囊充气的第1、2、3、4、5h及实验结束后分别取套囊压迫处气管黏膜送病理。结果 C组气管黏膜病理学改变程度较重:其中1度损伤7例,明显少于A及B组;4度损伤4例,明显多于A及B组;B组不同程度损伤的例数与A组比较差异无统计学意义(P>0.05)。结论 20-25mmHg的气管导管套囊内压对气管黏膜损伤较轻,可作为适宜的术中套囊内压力参考值范围。
Objective To observe the effect of pressure on the tracheal mucosa in different sets of cuff by animal ex periments,and explore the optimal pressure range of the protective effects of the tracheal mucosa. Methods 60 healthy rabbits were randomly divided into 3 groups, 20 rabbits in each group, 3 3. S months old, male and female, and the weight were about 3-4 kg. That is,low pressure group (group A) :the pressure of cuff was 20 mmHg,middle pressure group (group B),the pressure of cuff was 25 mmHg,high pressure group(group C),the pressure of cuff was 30 mm Hg. The type of tracheal catheter included:ID3.5 mm, (Ic Covidien). Take the pathological sample on the time point of the 1,2,3,4,5th hour and the time point after end of the experiment for analysis respectively. Results the pathological changes in group C were more serious. 7 cases with 1 degree of injury in group C were significantly less than that in group A and group B, 4 cases with 4 degrees of injury in group C were significantly more than that in group A and group B,and there was no significant difference between group A and group B (P〉0.05). Conclusion The tracheal tube cuff with the pressure of 20-25 mmHg can cause the most light injury on the tracheal mucosa, so the pressure of 20-25 mmHg can be used as the reference value range of the cuff.
