大黄煎剂保留灌肠对轻微型肝性脑病患者血清代谢物谱的影响

Effect of Dahuang Decoction(大黄煎剂) Retention Enema on Serum Metabolite Profiling in Patients with Minimal Hepatic Encephalopathy

目的探讨大黄煎剂治疗轻微型肝性脑病(MHE)患者的可能作用机制。方法将29例MHE患者随机分为大黄煎剂组15例和安慰剂组14例。大黄煎剂组给予大黄煎剂200 ml保留灌肠,安慰剂组给予无菌注射用水200 ml保留灌肠。两组均每天1次,7天为1个疗程。治疗前后检测血清样本中的代谢物,用主成分分析(PCA)和偏最小二乘法判别分析(OPLS-DA)分析两组患者治疗前后血清样本中的代谢物变化情况。结果 PCA分析显示,大黄煎剂组及安慰剂组治疗前后血清代谢轮廓的分组趋势不明显,有较多重叠,模型参数R^2X分别为0.393和0.509,模型的解释能力欠佳;OPLS-DA分析显示,大黄煎剂组模型参数R^2X=0.280,R^2Y=0.954,Q^2Y=-0.097;安慰剂组模型参数R^2X=0.397,R^2Y=0.941,Q^2Y=-0.605,模型建立失败。两组治疗前后MHE患者的代谢物谱均无明显差异。结论大黄煎剂对MHE患者的血清代谢物谱无明显影响。

Objective To explore possible mechanism of Dahuang Decoction in treating minimal hepatic encephalopathy（MHE）.Methods Randomly divided 29 cases of patients with MHE into the Dahuang Decoction group（15 cases） and the placebo group（14 cases） The Dahuang Decoction group was given Dahuang Decoction 200 ml for retention enema.The placebo group was given sterile water injection 200 ml for retention enema.Both once per day,lasted for 7 days as a course.Examined metabolite in serum samples before and after treatment.Analyzed changes of metabolite in serum samples of patients before and after treatment using principal component analysis（PCA） and partial least square discrimination analysis（OPLS-DA）.Results PCA indicated that grouping trend of serum metabolite profile of the placebo group and the Dahuang Decoction group before and after treatment was not obvious and had much overlap.Model parameter R X was respectively 0.393 and 0.509.Model interpretability was not good enough.OPLS-DA showed that model parameter of the Dahuang Decoction group R^2X = 0.280,R^2Y = 0.954,Q^2Y =-0.097;Model parameter of the placebo group R^2X =0.397,R^2Y =0.941,Q^2Y =-0.605.The model was not established successfully.Serum metabolite profiling of patients with MHE in two groups before and after treatment had no obvious difference.Conclusion Dahuang Decoction has no obvious effect on serum metabolite profiling of patients with MHE.
