超声引导冷循环微波热消融在乳腺多发纤维腺瘤治疗中的应用价值 被引量:8

摘要 目的探讨超声引导冷循环微波消融术(MWA)在乳腺多发纤维腺瘤治疗中的应用价值。方法选择确诊的16例乳腺多发纤维腺瘤患者(37个肿瘤),年龄25~37岁,平均年龄31.5岁。进行MWA治疗,观察治疗效果及治疗前后超声声像图变化。结果彩色多普勒超声显示37个肿瘤内血流信号均消失;治疗时间6~17 min,平均治疗时间10.4 min(标准差4.6 min)。16例均无严重并发症发生。治疗前肿瘤体积0.34~4.18 cm^3,平均肿瘤体积1.76 cm^3(标准差1.55 cm^3);治疗后1个月,超声随诊显示肿瘤体积轻度增大,回声减低,瘤体边界模糊;治疗后3、6、12个月超声检查显示肿瘤体积较治疗前逐渐缩小,缩小率分别为(25.1±6.1)%、(45.4±7.4)%、(73.6±6.3)%。结论超声引导MWA治疗安全、微创,适用于不愿接受手术治疗的乳腺多发纤维腺瘤患者。 Objective To study the clinical value of microwave ablation(MWA) in the multiple breast fibroadenoma by ultrasound guidance. Methods A total of 16 female patients with multiple breast fibroadenoma(37 breast neoplasms) were enrolled, which aged 25 - 37 years old with mean age of 31.5 years old. They were treated with MWA, curative effects and sonographic changes before and after treatment were recorded. Results Thirty-seven fibroadenomas hemodynamie signal were disappeared by color Doppler flow imaging(CDFI) after treatment. The treatment time was 6 - 17 minutes with mean of 10.4 minutes(standard deviation was 4.6 minutes), and without severe complications. The neoplasms volume was (0.34 - 4.18) cm3 with mean of 1.76 em3(standard deviation was 1.55 em3) before treatment. After treatment 1-month, the US showed that echo reduced with border un-sharply. The neoplasms volume were obviously diminished(P 〈 0.05) at 3, 6, 12 months after treatment, and diminished rate was (25.1 ± 6.1) %, (45.4 ± 7.4) % and (73.6 ±6.3) %, respectively. Conclusion It is demonstrated that MWA by ultrasound guidance is safety with minimal invasive, which is especially for operation-intolerant patients with multiple breast fibroadenoma.
