两种注射治疗方法对突发性聋的疗效分析

Efficacy Analysis of Two Different Injection Methods for the Treatment of Sudden Sensorineural Hearing Loss

目的观察耳后注射与鼓室内注射甲泼尼龙这两种治疗方法对突发性聋的疗效，为临床选择突发性聋治疗方案提供参考。方法回顾性分析2009年6月～2015年3月收治的突发性聋患者268例，按照治疗方式不同分为耳后注射组（135例）及鼓室内注射组（133例）。对两组患者治疗前后听阈水平、治疗效果、一般治疗情况以及随访情况进行观察与对比。结果治疗前后两组听阈无显著性差异（P〉O．05）。耳后注射组痊愈率为57．04％（77／135），远高于鼓室内注射组痊愈率39．10％（52／133），差异有统计学意义（P〈O．05）。耳后注射组总有效率为82．96％（112／135），鼓室内注射组总有效率为79．70％（106／133），两组无显著性差异（P〉O．05）。耳后注射组平均治疗时间、注射次数及不良反应发生率均低于鼓室内注射组，差异有统计学意义（P〈O．05）。治疗后6个月随访结果显示，耳后注射组病情复发率、有耳鸣及耳堵塞感患者的比例均低于鼓室内注射组，差异有统计学意义（P〈O．05）。结论耳后注射甲泼尼龙治疗突发性聋疗程短、疗效好，创伤及副作用小，安全性较高，值得临床推广应用。

Objoctlve To explore the efficacy of retroauricular and tympanic injection in the treatment of sudden sensorineural hearing loss in order to provide evidences for the selection of treatment methods. Methods 268 patients with sudden sensorineural hearing loss were retrospectively analyzed and divided into retroauricular injection group （135 cases） and tympanic injection group （133 cases）. Then the hearing thresholds, efficacy, general treatment conditions and follow- up information of the two groups were detected and compared. Results The hearing thresholds of the two groups before and after treatment had no significant difference （P〉0.05）. The cure rate of retroauricular injection group was 57.04%, which was significantly higher than that of tympanic injection group （39.10%） （P〈0.05）. The total effective rate of the two groups had no significant difference （P〉0.05）. The average treatment time, frequeney of injection and adverse reaction rate of retroauricular injection group were significantly lower than tympanic injection group（P〈0.05）. The followed-up results showed that the recurrence rate and percentage of tinnitus and ear fullness of retroauricular injection group were significantly lower than tympanic injection group （P〈0.05）. Conclusion Retroauricular injection of methylprednisolone has advantages of short treatment course, good efficacy, less pain and adverse reaction in the treatment of sudden sensorineural hearing loss, so it is better for clinical application.
