活血止痛膏外敷结合冷疗及弹性固定对腕关节扭挫伤临床疗效的影响

The Clinical Effect of Huoxue Zhitong Ointment Combined with Cryotherapy and Elastic Fixation of Wrist Sprain

目的:探讨活血止痛膏外敷结合冷疗及弹性固定对腕关节扭挫伤临床疗效的影响。方法:将2013年5月至2014年12月急诊诊治的腕关节扭挫伤患者随机分为活血止痛膏外敷结合冷疗及弹性固定的治疗组以及单纯冷疗及弹性固定的对照组。参照《中医病证诊断疗效标准》中腕关节扭挫伤的疗效标准评估临床疗效。采用视觉模拟评分(Visual Analogue Score,VAS)评估疼痛改善情况。采用Cooney腕关节评分评估腕关节功能。结果:1)治疗8d后治疗组显效10例,有效23例,优于对照组的显效2例,有效27例,差异有统计学意义(P<0.01);治疗20d后治疗组显效25例,有效9例,优于对照组的显效10例,有效24例,差异有统计学意义(P<0.01)。2)两组组内治疗8d后及治疗20d后VAS评分与治疗前相比,差异有统计学意义(P<0.01);治疗组治疗8d后及治疗20d后VAS评分显著低于对照组,差异有统计学意义(P<0.01)。3)治疗组组内治疗8d后及治疗20d后Cooney腕关节评分各维度与治疗前相比,差异有统计学意义(P<0.01);对照组组内治疗8d后Cooney腕关节评分中疼痛、活动范围、握力与治疗前相比,差异无统计学意义(P>0.05),对照组组内治疗8d后Cooney腕关节评分中功能状况和治疗20d后Cooney腕关节评分各维度与治疗前相比,差异有统计学意义(P<0.05或P<0.01)。治疗组治疗8d后及治疗20d后Cooney腕关节评分各维度显著高于对照组,差异有统计学意义(P<0.01)。结论:活血止痛膏外敷结合冷疗及弹性固定治疗腕关节扭挫伤,能更快缓解腕关节疼痛,改善腕关节功能,临床疗效满意。

Objective：To explore the clinical effect of Huoxue Zhitong ointment combined with crotherapy and elastic fixation in treating wrist sprain. Methods：From May 2013 to December 2014, the patients of wrist sprain were randomly divided into the treatment group treated with Huoxue Zhitong ointment combined with cryotherapy and elastic fixation,and the control group treated with combined with cryotherapy and elastic fixation only. The clinical efficacy was evaluated accord- ing to the TCM syndromes diagnostic criteria of wrist sprain. VAS score was used to evaluate the improvement of pain, and wrist joint function was evaluated by cooney wrist score. Results： After 8 days treatment, 10 cases were markedly improved and 23 cases were effective in the treatment group and 2cases were markedly improved and 27 cases were effective in the control group, which shows statistically significant difference（P〈0. 01）. After 20 days treatment, 20case were markedly improved and 9cases were effective in the treatment group and 10 cases were markedly improved and 24cases were effective in the control group,which shows statistically significant difference（P〈0.01）. Compared with that before treatment,the VAS of the two groups after 8and 10 days treatment was improved and the difference was signifieant（P〈 0.01）. After treating for 8 and 20 days,the VAS of the treatment group was significantly lower tha that of the control group（P〈0.01 ）. After 8 and 20 days treatment, the Cooney score of treatment group was improved in each dimension and the difference was significant（P〈0.05 or P〈0. 01）. The Cooney score of control group in pain, range and grip strength showed no significance compared with that before treatment after treating for 8 days（P〉0.05）. The Conney score of the treatment group was significantly higher than the control group in each demension after 8 and 20 days treatment（P〈0.01）. Conclusion.. Huoxue Zhitong ointment external aplieation combined with cryotherapy and elastic fixation can quickly relieve wrist pain and improve the wrist function,and the curative effect is satisfied.
