摘要
目的探究慢性心力衰竭患者左心室流体力学采用超声血流向量成像分析的应用价值。方法将接收的慢性心力衰竭患者30例(观察组)及同期30例健康者(对照组)作为研究对象。测定两组患者射血-充盈血流逆转间期(FRI)、射血-充盈血流逆转流率(FRR)、校正射血-充盈血流逆转间期(CFRI)、左心室射血期峰值流率(FRRE),并比较两组患者左心室流体力学的参数差异。结果慢性心力衰竭左心室流体力学FRR与左FRRE,明显低于对照组,CFRI与FRI明显高于对照组(P<0.05);心肌做功指数(Tei)指数和FRI、CFRI呈正相关,短率(FS)、心博出量(SV)、左心室射血分数(LVEF)、心输出量(CO)、心指数(CI)与FRI呈负相关;FS、LCO、SV、CI、VEF与CFRI呈负相关;Tei指数与FRR呈负相关;EF、SV、FS、CI、CO与FRR呈正相关,Tei指数与FRPE呈负相关,FRPE与FS、LVEF、CO、CI、SV呈正相关,CFRI、FRI、FRPE、FRR与LVESV、LVEDV并无明显相关性。结论慢性心力衰竭患者应用超声血流向量成像能直观地显示及定量分析其左心室流体力学变化,且左心室内血流方向逆转的效率能作为判断其治疗效果的指标之一。
Objective To explore using the application value of ultrasound blood flow vector imaging analysis the left ventricle in patients with chronic heart failure of fluid mechanics. Methods 30 cases of patients with chronic heart failure and 30 cases of healthy subjects were chose as the research object,including chronic heart failure patients as observation group,healthy subjects as control group. To determination 2 groups of patients with blood flow reversal interphase( FRI),ejection-filling flow reversal flow rate( FRR),calibration period between ejection-filling flow reversal( CFRE),left ventricular ejection period peak flow rate( FRRE),and compare the differences of left ventricular fluid mechanics parameters between two groups of patients with. Results Chronic heart failure in the left ventricle and the FRR of fluid mechanics FRRE were significantly lower than the control group,CFRI with FRI were significantly higher than the control group,the comparison between groups,statistically significant difference( P〈0. 05); Tei index and FRI,CFRI were positively correlated,FS,SV,LVEF,CO,CI and negatively correlated with FRI; FS,LCO,SV,CI,VEF and negatively were correlated with CFRI; Tei index and FRR were negatively correlated; EF,SV,FS,CI,CO and FRR were positively correlated,and negatively correlated with FRPE Tei index,FRPE and FS,LVEF,CO,CI and SV were positively correlated,CFRI,FRI,FRPE and FRR,LVESV and LVEDV had no significant correlation. Conclusion Patients with chronic heart failure in application of ultrasonic blood flow vector imaging and quantitative analysis could intuitively show the left ventricle changes in fluid mechanics,and indoor left heart blood flow direction reversal efficiency could tell its therapeutic effect as one of the indicators.
