摘要
目的:探讨以肝脏、脾脏CT体积评估门静脉压力的可行性。方法:正常成年比格犬30只,随机分为门静脉高压症(PHT)组22只,对照组8只。PHT模型采用门静脉主干缩窄联合硫代乙酰胺溶液喂养诱导建立。应用16排螺旋CT测量2组比格犬肝脏、脾脏体积,此后开腹,通过玻璃水柱法直接测定门静脉压力。结果:PHT组比格犬肝脏体积[(270.253±10.445)cm3]较对照组[(414.168±10.702)cm3]缩小(P<0.001),而脾脏体积[(68.498±17.286)cm3]与对照组[(63.196±12.212)cm3]相比,差异无统计学意义(P=0.458)。PHT组比格犬肝脏体积与门静脉压力呈正相关(r=0.716,P<0.001),而脾脏体积与门静脉压力无关(r=0.022,P=0.920)。结论:比格犬PHT模型的肝脏CT体积可较敏感地反映门静脉压力。
Aim: To discuss the feasibility of assessment of portal vein pressure according to the liver and spleen volume by multi-slice spiral CT. Methods: Twenty-two beagles were selected to establish model of portal vein hypertension( PHT) by combining with portal vein constriction induced by thioacetamide solution. Eight beagles were used as control.The liver volume and spleen volume were measured by 16 multi-slice spiral CT,and then,the portal vein pressure was detected by glass jets method. Results: The liver volume of beagles in PHT group was( 270. 253 ± 10. 445) cm3,which was lower than the controls [( 414. 168 ± 10. 702) cm3]( P〈0. 001). The spleen volume of beagles in PHT group was( 68. 498 ± 17. 286) cm3,which was( 63. 196 ± 12. 212) cm3 in the controls,and there was no significant difference between the two groups( P = 0. 458). The liver volume of beagles and portal vein pressure in PHT group were correlated( r =0. 716,P〈0. 001),while the spleen volume and portal vein pressure were not correlated( r = 0. 022,P = 0. 920). Conclusion: The liver volume measured by 16 multi-slice spiral CT could reflect portal vein pressure.
