鼻窦内窥镜手术治疗慢性鼻-鼻窦炎的临床分析

Improved symptom and nasal mucus cilium function in patients with chronic nasal-sinusitis after endoscopic sinus surgery

目的：探讨鼻窦内窥镜手术对慢性鼻-鼻窦炎患者的临床症状及鼻黏液纤毛清除功能的影响。方法：将2012年6月~2014年4月收治的100例慢性鼻-鼻窦炎患者,按照不同的手术治疗方法分成观察组与对照组,每组各50例患者,对照组实施传统手术治疗,观察组实施鼻窦内窥镜手术治疗,对两组患者的临床疗效及手术前后的主要临床症状评分,鼻黏液纤毛清除功能指标水平等进行比较。结果：观察组患者手术总有效率显著高于对照组,差异有显著性（P〈0.05）;而观察组术后6个月的嗅觉障碍、鼻塞、面部疼痛、鼻分泌物、头痛等主要临床症状评分均显著低于对照组,差异均有显著性（P〈0.05）;观察组患者术后2个月的糖精清除时间显著短于对照组,且鼻黏液纤毛清除速度及清除率均明显高于对照组患者,差异均有显著性（P〈0.05）。结论：采用鼻窦内窥镜手术治疗慢性鼻-鼻窦炎患者,可有效提高临床疗效、促进临床症状改善,并改善鼻黏液纤毛清除功能。

Objective： To observe the efficacies of endoscopic sinus surgery for improving the clinical symptoms and nasal mucus cilium clearance function in patients with chronic nasal-sinusitis. Methods： A total of 100 patients with chronic sinusitis treated in our hospital between June 2012 and April 2014 were equality allocated to observational group and control group by the surgical modality. The control group were treated with conventional surgery,and the observational group with endoscopic sinus surgery. The two group were compared regarding the scoring on clinical symptom improvement and nasal mucus cilium clearance function before and after surgery. Results： The total effective rate was significantly higher in the observational group than the control group（ P〈0. 05）,and the observational group scored significantly lower on the symptoms,including dysosmia,nasal congestion,facial pain,nasal discharge and headache by the sixth month after surgery（ P〈0. 05）,yet had shorter clearance time by saccharin test,better nasal mucociliary transport time and clearance rate two months after procedure（ P〈0. 05）. Conclusion： Endoscopic sinus surgery can be effective in patients with chronic nasal-sinusitis in improving their clinical symptoms and nasal mucus cilium clearance function.
