摘要 目的研究不同分冠法的两种高速涡轮机拔牙方法的疗效,并同常规劈冠方法进行对比。方法选择90例拔除下颌阻生第三磨牙患者,分成三组,各30例。观察A组采用"T+V"型涡轮机分牙冠加C型颈部增隙法;观察B组采用T型涡轮机分牙冠法;对照组采用劈冠法。统计患者手术时间、术中并发症及术后不良反应。结果观察A组手术时间比观察B组、对照组低,差异有统计学意义(P<0.05);观察A组术后疼痛及肿胀比观察B组、对照组低,差异有统计学意义(P<0.05);观察A组术中舌侧骨板骨折、断根残留发生率比观察B组、对照组低,差异有统计学意义(P<0.05)。结论高速涡轮机配合"T+V"型分冠加C型颈部增隙法,手术时间短,术中并发症低,术后不良反应少,效果更佳。 Objective To analysis the clinical efficacy of two different coronetomy techniques of high-speed turbine extraction methods,make a comparison with the conventional method of splitting crown.Methods Selected 90 cases of patients with mandibular third molar extraction patients were divided into three groups,each group had 30 cases,the observation group A used "T+V" shape of cutting crown by high speed turbine handpiece,combined with "C" type of cervical increasing gap to extract the impacted mandibular third molars,the group B used "T" shape of divided crown.30 cases as the control group used conventional split crown.The operative time length,intraoperative and postoperative complications were recorded.Results The operative time of group A was less than the observation group B and the control group.It was statistically significant(P〈0.05).Postoperative pain and swelling of the observation group A were lower than the observation group B and the control group(P〈0.05).the lingual plate fracture and the incidence of residual root pruning were lower than the observation group B and the control group(P〈0.05).Conclusion High-speed turbine with "T+V" type points plus C-neck crown gap method can shorten operative time,with less intraoperative lingual plate fracture and uprooting residual rate,and it has less postoperative adverse events and good effect.
