摘要
目的探讨子宫下段压迫缝合术在治疗前置胎盘的剖宫产术中大出血的有效性和安全性。方法将120例前置胎盘剖宫产术中发生产后出血的患者随机分为治疗组和对照组,每组各60例。对照组采用常规止血方法,治疗组采用子宫下段压迫缝合手术止血,观察两组止血效果及安全性。结果治疗组患者手术时间、术中出血量及输血量、产后出血率均明显低于对照组,差异具有统计学意义(P<0.05);两组子宫切除率相比不具有统计学意义(P>0.05)。治疗组患者并发症发生率为11.7%(7/60),对照组患者为26.7%(16/60),两组并发症发生率比较,差异具有统计学意义(P<0.05)。治疗组恶露干净时间及月经恢复时间较对照组均明显缩短,差异具有统计学意义(P<0.05)。结论子宫下段压迫缝合术在前置胎盘剖宫产术中可以取得较好的止血效果,简单易行,止血快,对患者损伤小,且并发症发生率低,值得临床推广。
OBJECTIVE To investigate the application effect of compression suture of uterine lower segment in large hemorrhage during cesarean section in placenta. METHODS 120 patients with postpartum hemorrhage after cesarean section were randomly divided into treatment group and control group, 60 cases in each group. The control group used conventional methods of hemostasis, and the treatment group treated with uterine lower segment compression suture. The effect and safety of the treatment were observed. RESULTS The operation time, bleeding volume, blood transfusion volume and postpartum hemorrhage rate in the treatment group were significantly lower than those in the control group, the difference was statistically significant(P〈O.05). There was no statistical significance of hysterectomy rate between the two groups(P〉0.05). The incidence of complications in the treatment group was 11.7% (7/ 60), the control group was 26.7%(16/60). There was statistical significance of the incidence of complications in the two groups (P〈 0.05). The treatment group of lochia time and menstrual recovery time was significantly shorter than the control group, the difference was statistically significant(P〈0.05). CONCLUSION Compression suture of uterine lower segment in large hemorrhage during cesarean section in placenta could obtain better hemostatic effect, it wass simple, easy, reliable, rapid and reliable, and no complications occurred. It wass worthy of clinical promotion in the treatment of uterine bleeding.
