腹腔镜辅助与开腹根治性全胃切除术治疗胃上部癌的临床效果比较

Comparative analysis of laparoscopic assisted gastrectomy and open gastrectomy treating upper gastric carcinoma

目的探讨腹腔镜辅助与开腹根治性全胃切除术治疗胃上部癌的临床效果。方法选取2012年2月至2013年6月经临床确诊为胃上部癌并于解放军第三军医大学第三附属医院进行治疗的患者100例，将患者完全随机分为观察组和对照组，各50例。对照组采用开腹根治性全胃切除术，观察组采用腹腔镜辅助根治性全胃切除术。比较2组患者术中情况、术后恢复和并发症发生情况。结果与对照组比较，观察组患者手术时间更短，术中出血量更少，淋巴结清扫数目更多，幽门和脾门清扫范围更彻底，差异均有统计学意义[（168±27）rain比（216±37）min，（299±128）ml比（224±126）ml，（27±4）个比（19±3）个，（8．2±0．6）个比（4．3±0．3）个，（10．3±0．6）个比（5．2±0．4）个，均P〈0．05]。与对照组比较，观察组患者进流质食更早，下床活动和术后排气时间均明显提前，同时术后住院时间缩短，差异均有统计学意义[（13±5）h比（79±15）h，（36±12）h比（52±14）h，（72±10）h比（98±14）h，（5．4±1．8）d比（14．3±10．2）d，均P〈0．05]。2组患者术后并发症发生率比较，差异无统计学意义（P〉0．05）。结论与传统开腹术比较，采用腹腔镜辅助的方式对胃上部癌患者进行治疗具有术期短、术中出血量少、癌灶清除彻底、预后良好等优点。

Objective To compare the effects of laparoscopic assisted gastrectomy and open gastrectomy on upper gastric carcinoma. Methods Totally 100 patients with upper gastric carcinoma from February 2012 to June 2013 were randomly divided into observation group （50 cases） receiving total laparoscopic assisted radical gastrectomy and control group （50 cases） receiving open radical total gastrectomy. The intraoperative conditions, postoperative recovery and complications were observed between the two groups. Results Compared with control group, observation group showed significantly shorter operation time, less blood loss, more dissected lymph nodes and more dissected extent [ （168 ±27） min vs （216 ±37） min, （299 ± 128） ml vs （224 ± 126） ml, （27±4） vs （19±3）, （8.2±0.6） vs （4.3＋-0.3）,（10.3±0.6） vs （5.2±0.4）] （allP〈0.05）. The time of taking liquid food, ambulation, postoperative exhaust and length of hospital stay were significantly improved in observation group compared with those in control group [ （13 ±5） h vs （79 ± 15） h, （36 ± 12） h vs （52 ± 14） h, （72 ± 10） hvs （98±14） h, （5.4±1.8） dvs （14.3±10.2） d] （allP〈0.05）. Conclusion Compared with open radical total gastrectomy, laparoscopic assisted radical gastrectomy has obvious advantages such as shorter duration of operation, less blood loss, more complete lesion clearance, better healing.
