HPAG内镜象与中医证型的相关性研究

Association between endoscopic manifestations and TCM syndrome types of Hp-associated gastritis

目的:探究Hp相关性胃炎(HPAG)内镜象与中医证型的相关性。方法:将84例HPAG患者的基本情况、辨证分型与胃镜表现进行数据录入,并采用SPSS 17.0统计软件处理分析。结果:HPAG的发病与性别、年龄、诱发因素及是否外出就餐有关。浅表性胃炎主要以脾胃湿热证和肝胃郁热证为主,萎缩性胃炎主要为胃络瘀阻证和胃阴不足证。HPAG不同中医证型与黏膜表现及舌质、舌苔具有一定的相关性。脾胃湿热证黏膜多表现为糜烂及充血,多见红舌、黄腻苔;肝胃郁热证黏膜改变以糜烂为主,多见红舌、黄苔;胃络瘀阻证舌象多表现为暗红舌;胃阴不足证的舌苔少或见剥脱。结论:HPAG患者的内镜表现与中医证型具有相关性。

Objective： To investigate the association between endoscopic manifestations and traditional Chinese medicine（ TCM） syndrome types of Hp- associated gastritis（ HPAG）. Methods： The general information,TCM syndrome types,and gastroscopic manifestations of 84 HPAG patients were recorded,and SPSS 17. 0 was applied for statistical analysis. Results： The onset of HPAG was associated with sex,age,inducing factors,and whether the patient ate outside. The patients with superficial gastritis mainly had the syndromes of damp heat in the spleen and the stomach and heat accumulation in the liver and the stomach,and those with atrophic gastritis mainly had the syndromes of static blood in stomach collaterals and stomach yin insufficiency. The different TCM syndrome types were associated with mucosal manifestations,tongue nature,and coating on the tongue. In the syndrome of damp heat in the spleen and the stomach,erosion and congestion occurred in the mucosa,with the presence of red tongue and a yellow greasy tongue coating; in the syndrome of heat accumulation in the liver and the stomach,mucosal changes were mainly erosion,and red tongue and a yellow tongue coating were often seen; in the syndrome of static blood in stomach collaterals,tongue manifestation was mainly dark red tongue; in the syndrome of stomach yin insufficiency,there was less tongue coating and peeling of the tongue coating occurred occasionally. Conclusion： In the patient with HPAG,the endoscopic manifestations are associated with TCM syndrome types.
