静脉药物配置中心不合理医嘱分析

Analysis of Irrational Medical Orders in the Pharmacy Intravenous Admixture Service of Beijing Luhe Hospital Affiliated to Capital Medical University

目的:评价首都医科大学附属北京潞河医院(以下简称"我院")静脉药物配置中心(pharmacy intravenous admixture service,PIVAS)的不合理医嘱情况,为促进临床合理用药提供参考。方法:查阅2014年6—12月我院PIVAS药师审核出的303份不合理医嘱,并进行归类、汇总、分析。结果:我院PIVAS的不合理医嘱主要集中在溶剂选择不合理(占55.12%)和溶剂用法、用量不合理(占15.18%)两方面,所涉及的药品主要是中药注射剂(占33.66%)和抗菌药物(占22.44%)。经药师审核并及时反馈,大部分不合理用药医嘱得以纠正。结论:药师对PIVAS的不合理医嘱进行审核,可促进临床医师合理用药,保障患者用药安全。

OBJECTIVE：To evaluate the irrational medical orders in the pharmacy intravenous admixture services（ PIVAS） of Beijing Luhe Hospital Affiliated to Capital Medical University（ hereinafter referred to as＂our hospital＂）,and to provide reference for the promotion of rational drug use in clinic.METHODS：303 irrational medical orders which were audited by pharmacists in PIVAS of our hospital from Jun.to Dec.2014 were statistically classified,summarized and analyzed.RESULTS：The irrational medical orders were mainly occurred in the inappropriate selection of solvent（ 55.12%） and improper usage or dosage of solvent（ 15.18%）,mainly included traditional Chinese medicine injections（ 33.66%） and antibiotics（ 22.44%）.Most of the irrational medication orders were corrected through the audit and timely feedback of the pharmacists.CONCLUSIONS：Clinical rational drug use and medication safety of the patients could be improved by the audit of irrational medical orders of the pharmacist in PIVAS.
