小切口非超声乳化白内障摘除术核出囊袋的手术体会

Experience of Small Incision Non Phacoemulsification Cataract Extraction With Nucleus Out of Pocket

目的探究小切口非超声乳化白内障摘除术核出囊袋的手术体会。方法收集2014年1～7月在我院行小切口非超声乳化白内障摘除术的186例白内障患者，由同一位医师主刀完成小切口非超声乳化白内障摘除术，用截囊针行前囊膜环形撕囊，撕囊直径5．5～6m扎针对不同大小的晶体核，采用旋转晶体核出囊口、通过“洗核”缩小晶体核体积后旋转出囊口、扩大CCC（连续环行撕囊）直径后旋转出核等方法，将核娩出囊袋。结果在将晶体核旋出囊袋口的过程中，顺利出囊口182例，悬韧带断裂、后囊破裂4例。结论在小切口非超声乳化白内障摘除术中，因为晶体核需整体移出囊袋，需要囊袋口与晶体核的直径相匹配，才能保证手术安全。

Objective To explore small incision phacoemulsification cataract surgery experience a nuclear enucleation pouch. Methods 186 cases of small incision non phacoemulsification cataract extraction of cataract patients from January to July 2014 in our hospital, leading to complete by the same physician small incision non phacoemulsification cataract extraction, using cutting needle anterior capsule membrane of circular capsulorhexis, capsulorhexis 5.5~6 mm in diameter, according to different size of crystal nucleus, USES the rotating crystal nucleus capsule, through the ＂nuclear＂ crystal nucleus volume rotating sac outlet, expand the CCC（continuous curvilinear capsulorhexis） diameter after rotation method such as nuclear, nuclear hydrodissection themselves. Results In the crystal nuclear spin out of the capsule in the process of the mouth, well out of the capsule 182 cases, suspensory ligament rupture, posterior capsular rupture in 4 cases. Conclusion In small incision non phacoemulsification cataract extraction, because of the crystal nucleus to remove pouch, overall need capsule mouth match the diameter of the crystal nucleus, to ensure the safety of the operation.
