阿托伐他汀对不稳定性心绞痛患者vWF、sVCAM-1的影响

Effects of atorvastatin on vWF and sVCAM-1 in patients with unstable angina pectoris

目的探讨阿托伐他汀对不稳定性心绞痛患者血管性血友病因子(v WF)、血管细胞黏附因子(s VCAM-1)的影响,为临床诊断治疗提供参考。方法将142例不稳定性心绞痛患者随机分为对照组(n=71)和观察组(n=71);对照组给予常规西药治疗,治疗组在对照组治疗基础上加用阿托伐他汀治疗。比较两者患者治疗前后血总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C),内皮舒张功能指标NO浓度、反应性舒张程度、静息状态直径,v WF、s VCAM-1指标变化情况。结果治疗后观察组TC、TG、LDL-C显著低于对照组,而HDL-C水平高于对照组,且差异有统计学意义(P<0.05);观察组治疗后NO浓度、反应性舒张程度显著高于治疗前,且显著高于对照组治疗后,差异有统计学意义(P<0.05);治疗后观察组v WF、s VCAM-1显著低于对照组,且差异有统计学意义(P<0.05)。结论阿托伐他汀能显著改善不稳定心绞痛患者血脂水平及血管内皮功能,延缓冠状动脉粥样硬化进程。

Objective To investigate the effects of atorvastatin on von Willebrand factor（ v WF） and soluble vascular cell adhesion molecule-1（ s VCAM-1） in patients with unstable angina pectoris. Methods A total of 142 patients with unstable angina pectoris weredivided into trial group and control group at random（ n = 71 / group）. The control group was treated with routinemedicine. The trial group was additionally treated withatorvastatin. The two groups were compared in the aspects of bloodtotal cholesterol（ TC）,triglyceride（ TG）,low density lipoprotein cholesterol（ LDL-C）,high density lipoprotein cholesterol（ HDL-C）,NO（ index reflecting endothelial diastolic function）,reactive diastolic degree,resting diastolic diameter,v WF and s VCAM-1. Results The trial group reached significantimprovements in TC,TG,LDL-C and HDL-Cafter treatment when compared with the control group（ P〈0. 05）. The nitric oxide concentration and the reactive diastolic degree of the trial group after treatmentwere significantly higher than thatbefore treatment and thanthat of the control group after treatment（ P〈0. 05）. After treatment,v WF and s VCAM-1were significantly lower in the trial group than in the control group（ P〈0. 05）. Conclusion Atorvastatin can significantly improve the blood lipid levels and vascular endothelial function in patients with unstable angina pectoris. At the same time itdelays the progressof coronary atherosclerosis.
