胰宁消积汤联合化疗治疗中晚期胰腺癌39例临床观察

A Clinical Observation on Yining Xiaoji Soup combined With Chemotherapy for Middle-late 39 Cases of Pancreatic Cancer

[目的]观察胰宁消积汤联合化疗治疗中晚期胰腺癌疗效。[方法]使用前瞻性设计方法,对39例中晚期胰腺癌住院患者使用胰宁消积汤联合化疗,吉西他滨1000mg/m 2+0.9%氯化钠250mL,静滴,第1、8d,在30~60min内静滴完毕;顺铂25mg/m2+0.9%氯化钠250mL中静滴,第1~3d,先静滴顺铂,顺铂滴完后再静滴吉西他滨。化疗前30 min,托烷司琼(齐鲁制药有限公司,国药准字H20050535 5mg/支5mg,静注。胰宁消积汤(黄芪、党参各30g,白术20g,茯苓、白花蛇舌草、丹参各15g,生薏仁20g,莪术12g,土茯苓20g,枳壳12g,白英15g,鸡内金12g,夏枯草、苦参、元胡、郁金、柴胡各15g;恶心呕吐重加竹茹、半夏各15g;大便秘结加大黄^(后下)10g;发热加石膏30g,知母12g)。连续治疗21d为1疗程。观测临床症状、临床受益反应、KPS评分、毒副反应。连续治疗2疗程,判定疗效。[结果]CR 0例,PR14例,SD 20例,PD 5例,总有效率(cR+PR)35.90%(14/39),初治有效率45.00%(9/20),复治21.05%(4/19)。临床12例1,2级疼痛治愈,5例3级疼痛、4例4级疼痛减轻到2级,疼痛缓解率为63.64%。19例KPS评分较治疗前提高20分,体力状态改善率为48.72%。15例患者体重增加≥5kg,体重改善率为38.46%。[结论]胰宁消积汤联合化疗治疗中晚期胰腺癌,疗效满意,无严重不良反应,值得推广。

[Objective]To Yining Xiaoji soup combined chemotherapy,the curative effect of treatment of middlelate pancreatic cancer.[Methods]Using prospective design method,39 cases of hospitalized patients with middlelate pancreatic cancer using Yining Xiaoji soup treatment with combination chemotherapy,Gemcitabine 1000mg/m2＋0.9% sodium chloride 250 ml,static drop,1,8 d,the static drop out within 30 to 60minutes;Cisplatin,25mg/m2＋0.9% sodium chloride static drop in 250 ml,1~3d,first static drops of cisplatin,cisplatin,drip off again after static drops of gemcitabine.30 min before chemotherapy,tropane Si Qiong（qilu pharmaceutical co.,LTD.,approved by H20050535 5 mg/branch）5mg,static note.Yining Xiaoji Soup：Huangqi,Dangshen,each30 g,Baishu 20 g,Fuling,Baihuasheshecao,Dan shen each 15 g,Shengyiren 20 g,Eshu 12 g,Tufuling 20 g,Zhike 12 g,Baiying 15 g,Jineijin 12 g,Xiakucao,Kushen,Yuanhu,Yujin,Chaihu,each 15g;Nausea and vomiting with Zhuru,Banxia each 15 g;Constipation increase Dahuang10g（afte r）;fever with Shigao 30 g,Zhimu 12 g.21 d for a period.Observation of clinical symptoms,clinical benefit response,toxic and side reaction.Continuous treatment 2 period of treatment,judge curative effect.1 course of treatment.Observation of clinical symptoms,clinical benefit response,KPS score,toxic and side reaction.Continuous treatment 2 period of treatment,judge curative effect.[Results]CR 0 cases,PR 14 cases,SD 20 cases,PD 5 cases,the total effective rate（CR＋PR） 35.90%（14/39）,In the early cure effective rate was 45.00%（9/20）,The retreatment of 21.05%（4/19）.Clinical 12 patients were cured pain level 1,2,5 cases of level 3,and 4 cases of pain reduction in pain level 4 to level 2,the pain relief rate was 63.64%.19 cases KPS score was 20 points,the physical state period was 48.72%.15 patients gain weight 5 kg or more,the weight period was 38.46%.[Conclusion]Yining Xiaoji soup combined chemotherapy treatment of locally advanced pancreatic cancer,curative effect is satisfied,no serious adverse reactions,worthy of promotion.
