辨证分型论治糖尿病周围神经病变

Treatment based on Syndrome Differentiation of Diabetic Peripheral Neuropathy

糖尿病周围神经病变是一个慢性病理演变过程,临床表现亦因人而异。辨证论治常收效甚捷,包括气血不荣、气阴两虚、阳虚寒凝、痰湿阻络、血脉瘀阻、肝郁气滞、脉络瘀毒等证,目前单一方剂的临床观察及疗效评价,虽能降低主观因素造成辨证认证不同导致疗效失真、控制干扰因素,但纳入样本均为某特定证型,难以规避对照治疗方案对此类特定样本疗效失真的误差;或仅辨病、非辨证对纳入样本均用某一方剂,导致此方剂疗效失真的误差。难以体现辨证论治、三因制宜的精髓。未来期待前瞻性随机大样本辨证论治、个体化治疗模式,结合(不结合)西药与单纯西药对照。

The diabetic peripheral neuropathy is a chronic pathological evolution process,clinical manifestations also vary from person to person.Treatment based on syndrome differentiation can get effect quickly,including Qi and blood are not honor,deficiency of Qi and Yin,deficiency of Yang and coagulated of cold,phlegm and dampness hindered the collaterals,stasis of blood and collaterals,depression of liver and stagnation of qi activity,stasis and toxin are in the collaterals.At present,the clinical observation and curative effect evaluation of a single prescription can reduce the bad effect of treatment and control the interference factors what because of the wrong syndrome differentiation,but the sample is a particular card type,difficult to avoid error of bad therapeutic effects,or all of samples use the same prescription without treatment based on syndrome differentiation lead to error of bad therapeutic effects.It could not reflect the marrow of treatment based on syndrome differentiation and treatment in accordance of three ways.Looking forward to prospective treatment based on syndrome differentiation with a large number of random samples,a presonal treatment model and combining with medicine（or not）campare with one kind of medicine.
