成人社区获得性肺炎降钙素原与中医证型相关性分析

Investigation on Relativity between Traditional Chinese Medical(TCM) Types and Procalcitionin(PCT) in Patients with Community-acquired Pneumonia(CAP)

[目的]探讨成人社区获得性肺炎(community-acquired pneumonia,CAP)患者降钙素原(procalcitonim,PCT)与中医证型之间的相关性、中医辨证分型的微观结构物质。[方法]回顾2012年09月1日至2014年12月31日于广东省中医院呼吸内科住院治疗并确诊CAP、年龄≥18岁的患者236例,获得降钙素原(PCT)及中医证型等资料,分析PCT与中医证型的相关性。[结果]成人CAP患者PCT在各中医证型中的分布存在差异,由高到低顺序分别为:肺胃热盛(0.44ng/m L)、痰热壅肺(0.20ng/m L)、风热犯肺(0.18ng/m L)、正虚邪恋(0.10ng/m L),其中肺胃热盛与痰热壅肺、风热犯肺、正虚邪恋比较,均具有统计学差异(P<0.05)。[结论]CAP患者随炎症轻重的不同而呈现不同的结果,说明PCT可以指导CAP中医的辨证分型以及辨证治疗,并用作判别疾病虚实的参考指标。

[Objective] To analyse the relativity between TCM types and PCT in patients with community-acquired pneumonia（CAP）. And probe the microscopic structure of TCM types. [Methods]Reviewed 236 cases（aged ≥18） with CAP who lived in Guangdong Provincial TCM Hospital from September 1,2012 to December 31,2014, all the cases had TCM types and PCT, study the distribution of TCM types and the relativity between TCM types and PCT.[Results]PCT mean descended sorting among groups were：syndrome of exuberant heat in lung and stomach（0.44ng/m L）〉accumulation of phlegm and heat in lung（0.20ng/m L） 〉attack of wind-heat on lung（0.18ng/m L）〉 syndrome of lingering pathogen due to deficient vital qi（0.10ng/m L）,compared with the other three syndromes,syndrome of exuberant heat in lung and stomach had significant differences（P〈0.05）.PCT varied significantly in TCM types in patients with CAP,syndrome of exuberant heat in lung and stomach was highest and syndrome of lingering pathogen due to deficient vital qi lowest.[Conclusion] CAP patients showed different results with different severity of inflammation that suggested that PCT could guide the TCM types and treatment of CAP,and could be used as a reference target for determining the actual condition of CAP. Further research needs to be conducted later.
