血清胱抑素C、肌酐、尿酸检测对尿糖阳性妊娠期妇女肾损伤的应用评价

The application evaluation of serum cystatin C, creatinine and uric acid detection for renal injury in pregnant women who positive of urine-glucose

目的：研究血清胱抑素C（serum cystatin C，Cys-C）、肌酐、尿酸对尿糖阳性妊娠期妇女肾损伤的早期诊断的临床价值。方法：选取我院产检尿糖正常中孕（在孕14—27周）妊娠妇女、晚孕（在孕28—36周）妊娠妇女和产检尿糖阳性的中孕、晚孕妊娠妇女各85例。血清胱抑素C的含量使用免疫比浊法检测，血清肌酐（SCr）、血清尿酸（UA）含量使用苦味酸比色法和尿酸比色法进行测量。比较正常中孕组、正常晚孕组、尿糖阳性中孕组、尿糖阳性晚孕组的各项指标，并进行统计学分析。结果：Cys-C在正常中孕组、正常晚孕组、尿糖阳性中孕组、尿糖阳性晚孕组的浓度分别为：（0．69±0．12）mg／L、（0．82±0．13）mg／L、（0．71±0．26）mg／L、（1．32±0．13）mg／L。且Cys-C在正常妊娠妇女与尿糖阳性妊娠期妇女中的变化有显著性差异，在肾损伤的早期检测中其比SCr和UA更加灵敏且可靠。结论：血清胱抑素C可作为尿糖阳性妊娠期妇女肾损伤的敏感指标，相较于血清肌酐和血尿素氮有更为重要的临床指导意义。

Objective ： To study the application value of the serum cystatin C（Cys-C） in pregnant women who test positive of urine-glucose sugar with renal injury. Methods ： research subjects：35 women in 14-27 weeks pregnancy（pregnant group）; 35 women in 28-36 weeks pregnancy（late pregnant group）; 35 women in 14-27 weeks pregnancy with positive of urine-glucose sugar（positive pregnant group）; 35 women in 28-36 weeks pregnancy with positive of urine-glucose sugar（positive late pregnant group）. The concentration of Cys-C were determined by nephelometry method. The concentration of serum creatinine （SCr） and serum uric acid （UA） were determined by picric acid colorimetric method and uric acid colorimetric method.Results ： The concentration of Cys-C of each group were （ 0.69 ± 0.12 ） mg/L （ 0.82 ± 0.13 ） mg/L （ 0.71 ± 0.26 ） mg/L, （ 1.32 ± 0.13 ） mg/L. The level of Cys-C had significantly difference in pregnant group and positive pregnant group. Conclusions ： Serum cystatin C has important significance in the early diagnostic in pregnant women who test positive of urine-glucose sugar with renal injury.
