自由体位对活跃期初产妇分娩结局的影响

The effect of free body position on the outcome of delivery in the early stage of active period

目的探讨自由体位对活跃期初产妇分娩结局的影响。方法选取医院218例行阴道分娩的初产妇,将其随机分为对照组与研究组各109例,其中对照组采取常规体位进行分娩,研究组采取自由体位进行分娩,观察2组活跃期胎先露下降速度、各产程时间、最终分娩方式、新生儿Apgar评分、新生儿体质量、产后24h出血量以及并发症。结果研究组胎先露下降速度明显优于对照组(P<0.05),产程时间明显缩短优于对照组(P<0.05)。研究组最终分娩方式阴道分娩率明显高于对照组(P<0.05),阴道助产以及剖宫产率也明显低于对照组(P<0.05)。2组新生儿Apgar评分、新生儿体质量对比,无明显统计学差异(P>0.05)。研究组产后24h出血量明显低于对照组(P<0.05),研究组并发症明显低于对照组(P<0.05)。结论活跃期初产妇采取自由体位可明显提高阴道分娩率,更有助于胎先露下降,从而缩短产程,降低并发症。

Objective To investigate the effect of free body position on the outcome of delivery in the early stage of active period. Methods 218 women with vaginal delivery were randomly divided into the control group and the research group,109 cases in each group,the control group was given routine body position for delivery,the study group to take a free body position for delivery. The active phase of the first dew decline rate,the time of birth,the final mode of delivery,neonatal Apgar score,neonatal weight,postpartum 24 h bleeding and complications of two groups were observed. Results The decline rate of the study group was significantly better than that of the control group（ P 〈 0. 05）,and the time of the study group was significantly shorter than that of the control group（ P 〈 0. 05）. The final set of mode of delivery vaginal delivery rate was significantly higher than that of control group（ P 〈 0. 05）,vaginal delivery and cesarean section rate was significantly lower than that of control group（ P 〈 0. 05）. Two groups of neonatal Apgar score,neonatal weight comparison,no significant statistical difference（ P 〉 0. 05）. The amount of postpartum hemorrhage of 24 h and complication rate in the study group were significantly lower than that of the control group（ P 〈 0. 05）. Conclusion In the active stage,the free posture can significantly increase the rate of vaginal delivery,which is more helpful to the decrease of fetal presentation,so as to shorten the production process and reduce the complications.
