糖化血红蛋白联合空腹血糖监测在妊娠期糖尿病筛查中的应用研究

THE APPLICATION OF COMBINING GLYCATED HEMOGLOBIN MONITORING AND FASTING BLOOD GLUCOSE MONITORING IN SCREENING GESTATIONAL DIABETES MELLITUS

目的探讨糖化血红蛋白联合空腹血糖监测在妊娠期糖尿病筛查中的应用效果。方法选取临沂市罗庄中心医院妇产科2013年4月-2015年8月间接收的妊娠期孕妇90例为对象进行研究,所有患者做HbAlc、FBG检查与OGTT并分析结果。结果 GDM组FBG、HbA1c显著高于对照组、GIGT组,HbA1c阳性检出率73.3%显著高于GIGT组36.7%、对照组20.0%,差异具有统计学意义（P〈0.05）;HbA1c检测特异度、诊断符合率高于OGTT与FBG,灵敏度略低于OGTT,HbA1c联合FBG检测可有效筛查GDM。结论糖化血红蛋白联合空腹血糖监测在妊娠期糖尿病筛查中有着较高的应用价值,可为GDM临床筛查、诊治提供有效依据,值得临床大力应用推广。

Objective To explore the effect of combining glycated hemoglobin monitoring and fasting blood glucose（FBG）monitoring in screening Gestational Diabetes Mellitus（GDM）.Methods All the 90 pregnant women treated in the Department of Obstetrics and Gynecology of Luozhuang Central Hospital from April,2013 to August,2015 received the tests of HbAlc and FBG and OGTT and the results were analyzed.Results In the GDM group,the levels of FBG and HbA1 cwere significantly higher than those in the contrast group and GIGT group.The positive rate of HbA1 cwas 73.3%,significantly higher than that（36.7%）in the GIGT group and taht（20.0%）in the contrast group（P 〈0.05）.The HbA1 cdetection rate of specific degree and the diagnostic accordance rate were higher than those the OGTT diagnosis and FBG detection,slightly lower than the sensitivity detection of OGTT.The combined use of HbA1 cand FBG detections can produce effective screening of GDM.Conclusion Combining glycated hemoglobin monitoring and fasting blood glucose monitoring has high clinical value in screening gestational diabetes mellitus.
