穴位埋线治疗原发性痛经疗效及安全性的Meta分析

Efficacy on Primary Dysmenorrhea Treated by Acupoint Catgut Embedding: a Meta Analysis

目的:系统评价穴位埋线对原发性痛经患者的疗效及安全性研究。方法:计算机检索Cochrane library、Pub Med、OVID、CNKI、CMCC、万方、维普数据库,检索年限均为建库至2015年3月,纳入穴位埋线治疗原发性痛经的随机或半随机对照试验,并对纳入文献进行质量评价和资料提取,采用Revman5.2软件对数据进行分析。结果:共纳入9个研究,704例患者。Meta分析结果显示穴位埋线在减轻患者疼痛方面优于对照组(WMD=-1.79,95%CI:-2.37^-1.21),在治疗3个月后及停止治疗3个月后,痛经症状改善与对照组比较差异无统计学意义(治疗3个月后WMD=-0.35,95%CI:-1.24~0.31;停止治疗3个月后WMD=-0.97,95%CI:-2.25,0.31),同时能够降低复发率(RR=0.08,CI95%:0.02,0.14)。结论:穴位埋线能有效减轻原发性痛经患者的疼痛,改善患者的痛经症状,降低痛经的复发,但由于纳入研究质量和数量限制,此结论尚需更多大样本、多中心、高质量的随机对照试验进行验证。

Objective： To evaluate the efficacy and security of acupoint catgut embedding on primary dysmenorrheal.Methods： We searched the Cochrane library,Pub Med,OVID,CNKI,CMCC,Wanfang Database and VIP Database for randomized or semi-random controlled trials of acupoint catgut embedding on primary dysmenorrheal by computers.Time had been limited to March 2015 since the database was estaldished.Evaluate the quality of the included studies and extracted the data,and then use the Rev Man5.2 software for meta analysis.Results： Nine researches were incorporated in the end,including 704 cases.Meta analysis showed： Acupoint catgut embedding could effectively relieve patients＇ pain compared with the control group（ WMD =-1.79,95% CI：-2.37 ～-1.21）;the dysmenorrhea symptom integral showed no difference compared with the control group after 3 months of intervention（ WMD =-0.35,95% CI：-1.24 ～ 0.31） and stopping intervention after three months（ WMD =-0.97,95% CI：-2.25,0.31）.At the same time it could also reduce the recurrence rate（ RR = 0.08,CI 95% ： 0.02,0.14） but we couldn＇t give a conculsion for the adverse reactions.Conclusion：Acupoint catgut embedding can relieve the pain of patients with primary dysmenorrheal to some extent and relieve the symptoms of dysmenorrheal,reduce the recurrence,but in terms of safety,it needs to be demonstrated.It is not enough comprehensive to evaluate the effectiveness and security of acupoint catgut embedding due to the quality and quantity of the included literatures.To validate the results still need high quality literatures.
