外固定支架-行损伤控制性治疗在创伤骨科修复中的应用 被引量:7

摘要 目的分析和观察外固定支架-行损伤控制性治疗在创伤骨科修复中的治疗效果。方法选取2012年6月-2014年6月骨科收治的60例创伤骨科患者,依照随机分配原则将所有患者分为实验组和对照组,每组30例。所有的患者均为骨盆骨折,对照组患者采用一期切开复位下行内固定治疗,实验组患者采用一期外固定支架治疗,随后在二期采用切开复位治疗。观察两组患者的死亡率、并发症发生率、手术时间、切口长度、骨折愈合时间、乳酸持续时间以及凝血功能恢复时间等进行比较。结果两组患者治疗效果对比分析,实验组和对照组患者的存活率分别为90%和66.67%,实验组患者的存活率明显高于对照组的(P〈0.05);实验组出现呼吸窘迫1例,感染2例,浆膜腔积液1例,均治愈出院;对照组患者出现呼吸窘迫2例,感染3例,浆膜腔积液3例,均治愈出院。实验组的并发症发生率为13.3%,对照组患者并发症发生率为26.7%,实验组患者并发症发生率明显低于对照组(P〈0.05);实验组患者的五项指标时间均明显短于对照组的(P〈0.05),差异具有统计学意义。结论外固定支架-行损伤控制性治疗在创伤骨科修复中的治疗效果显效,有利于提高患者的生存率,减少并发症的发生,缩短手术时间,促进骨头愈合,能够提高患者的生活质量。 Objective To analysis and observation of external fixator damage control treatment in Department of orthopedics and trauma repair treatment. Methods 60 cases of trauma patients in the Department of orthopedics from2012 June to 2014 June in the Department of orthopedics in our hospital,all patients were divided into experimental group and control group according to the random distribution principle,30 cases in each group. All patients had pelvic fractures,the control group were treated with an open reduction and internal fixation for the treatment of the experimental group downward,were treated by a period of fixation,followed by open reduction treatment in the two period. To observe the mortality,incidence of complications of the two groups,operation time,incision length,fracture healing time,lactate duration and coagulation function recovery time were compared. Results The results of comparative analysis of two groups of patients,the experimental group and the control group of patients survival rate were 90% and 66. 67%,the survival rate of the experimental group were significantly higher than the control group( P 〈0. 05); the experimental group appeared respiratory distress in 1 cases,2 cases of infection,1 cases of serous cavity effusion,the control group were cured; patients with respiratory distress in 2 cases,3 cases of infection,3 cases of serous cavity effusion,were cured. In the experimental group the incidence of complications was 13. 3%,patients in the control group the incidence of complications was 26. 7%,complications of patients in the experimental group was significantly lower than the control group( P 〈0. 05); five indicators of experimental group were significantly shorter than the control group( P 〈0. 05),the difference was statistically significant. Conclusions The external fixator to damage control effective therapy in the Department of orthopedics and trauma repair in the treatment effect,is beneficial to improve the survival rate of patients,reduce complications,shorten operation time,promote bone healing,can improve the quality of life of patients,is worthy of clinical promotion application.
