全麻和全麻联合硬膜外阻滞麻醉对胃癌根治手术后患者疼痛效果分析

The effect of general anesthesia and general anesthesia combined epidural block anesthesia on postoperative pain in gastric cancer radical gastrectomy for patients with gastric cancer

目的探究在对胃癌患者根治手术中应用全麻和全麻联合硬膜外阻滞麻醉患者术后疼痛效果。方法选择我院2014年1月至2015年3月收诊的108例胃癌患者,将所有患者按随机数字平均分为观察组与对照组,术中分别给予全麻和全麻联合硬膜外阻滞麻醉,记录两组患者麻醉前、中、后及术中生命体征指标、术后苏醒时间、拔管时间,术后VAS和术后不良反应率,对比分析。结果观察组在术中的心率、平均动脉压及静脉压等生命体征明显低于对照组,比较差异具有统计学意义(P<0.05)。观察组的醒来时间和拔管时间、术后1 h VAS、4 h VAS、8 h VAS等明显低于对照组,差异具有统计学意义(P<0.05)。观察组的术后不良反应率为29.63%,对照组的为46.30%,比较差异具有统计学意义(P<0.05)。结论全麻在术后容易发生重度疼痛,而应用全麻联合硬膜外阻滞麻醉在术后可以减轻患者的疼痛,效果明显,值得推广应用。

Objective This article was to explore the effect of general anesthesia and epidural block anesthesia on postoperative pain of patients undergoing radical operation for gastric cancer. Methods Chose 108 patients with gastric cancer in our hospital from January 2014 to March 2015 as the research subjects,the patients were randomly divided into observation group and control group. The patients were given general anesthesia or general anesthesia combined epidural anesthesia. The vital signs of life of patients in the groups before anesthesia,in the middle and after the anesthesia were recorded,as well as the postoperative recovery time,extubation time,postoperative VAS,and postoperative adverse reaction rate,and comparatively analyzed. Results The heart rate,mean arterial pressure and venous pressure were significantly lower in the observation group than in the control group,the differences were statistically significant（ P〈 0. 05）. The wake time,extubation time,postoperative VAS（ 1h,4h and 8h） of observation group were significantly lower than the control group,the differences were statistically significant（ P〈 0. 05）. The occurrence of postoperative adverse reactions of observation group was lower than that of control group. Conclusions Severe pain is easy to occur after general anesthesia,and the application of general anesthesia combined with epidural block anesthesia can reduce the pain of patients,the effect is obvious,it is worthy of popularization and application.
