宫腔镜下分段诊刮术对子宫内膜癌的诊断价值

Clinical value of fractional curettage in diagnosis of endometrial cancer under hysteroscopy

目的探讨宫腔镜下分段诊刮术对子宫内膜癌的诊断价值。方法回顾性分析我院2008年2月至2013年2月收治的子宫内膜癌患者179例,实验组90例与对照组89例,分别施行宫腔镜下分段诊刮术和单纯分段诊刮,比较两组术前与术后病理的分期分级判断的符合性、术前病理诊断子宫内膜癌组织学类型的符合率、比较两组冲洗液阳性率的差别并判断宫颈受累的可靠性。结果处理组术前临床分期与术后病理分期符合率、诊断子宫内膜癌的准确率、宫颈受累准确率均高于对照组,差异有显著性(P<0.05),通过细胞学检查判断知两组腹腔冲洗液阳性率无显著性差异(P>0.05),处理组3年存活率高于对照组(P<0.05)。结论宫腔镜下行分段诊刮术可以显著提高子宫内膜癌诊断的准确率和可靠性,提高患者预后水平,值得推广。

Objective To explore the clinical significance of fractional curettage in diagnosis of endometrial cancer under hysteroscopy. Methods Retrospectively analyzed 179 cases with endometrial cancer who received treatment in our hospital from February 2008 to February 2013. They were divided into observation group with 90 cases who were diagnosed with fractional curettage under hysteroscopy and control group with 89 cases who were diagnosed with simple sub-curettage. Then we observed these two groups ： stage agreement rate before and after the operation,the endometrial cancer histological types agreement rate before the operation,the flushing fluid positive rate and determined the reliability that the cervical been affected. Results Observation group＇s diagnosis agreement and cervical been affected rate are all higher than control group,the difference is significantly（ P〈 0. 05）. On the other hand,the flushing fluid positive rate has little difference（ P〈 0. 05）,3years survival probability of the treatment is better than the operation（ P〈 0. 05）. Conclusions Fractional curettage in diagnosis of endometrial cancer under hysteroscopy has pretty high accurate rate,and it affected the prognosis which is worthy of promotion.
