德宏州乡镇卫生院2013年免费主动提供艾滋病咨询检测服务人群HIV感染率及其影响因素

HIV Infection and Influencing Factors of the Patients Receiving PITC in Township Hospitals in Dehong Prefecture

目的了解德宏州乡镇卫生院主动提供艾滋病咨询检测服务（PITC）的人群中,艾滋病病毒（HIV）感染率及其影响因素。方法在德宏州全部54个乡镇卫生院,由经培训的医生对就诊者PITC：根据其危险因素选择对象,主动提供免费的HIV快速检测服务。收集并分析此人群的人口学特征及感染情况。结果 2013年共对64 105名就诊者PITC,其中来自芒市的占36.6%,来自门诊病人占73.7%;男性占41.0%;汉族占51.1%,傣族占30.7%;年龄＆gt;50岁占30.3%;农民占76.3%;小学文化占43.6%。发现HIV感染者179例,HIV感染率为0.3%,其中76例为新发现的感染者。HIV感染率瑞丽高达4.0%,其他4县（市）在0.1%~0.4%之间。住院病人的HIV感染率为0.5%,门诊病人为0.2%。Logistic多因素回归分析显示：瑞丽市感染率高于其他县市;住院病人感染率高于门诊病人;男性感染率高于女性;景颇族和傣族感染率高于汉族;19~30岁及31~50岁组感染率高于50岁以上组;文盲组感染率高于初中文化组。结论德宏州乡镇卫生院免费PITC工作在发现HIV阳性方面起到了重要作用,疫情严重的乡镇应进一步加强免费PITC工作。

Objective To learn the human immunodeficiency virus（HIV）epidemic among people receiving providerinitiated HIV testing and counseling（PITC）in Dehong prefecture.Method In all 54 township hospitals,trained doctors provided rapid HIV tests for free to the patients with risk behaviors.The sociodemographic characteristics and HIV testing results were collected and analyzed.Result A total number of 64 105 received PITC in 2013,with 36.6%from Mang City,73.7%from outpatients,41.0% male,51.1%and 30.7%for ethnic Han and Dai respectively.Most of the patients was over 50 years old accounting for 30.3%and peasants for 76.3%.43.6%of them received the education of primary school.179 HIV cases were detected,of which 76 were newly diagnosed.The overall HIV infection rate is 0.3%,with 4.0%in Ruili City,and 0.1% to 0.4% in other 4cities.The HIV infection rate of inpatients（0.5%）is higher than that of outpatients（0.2%）.Multiple logistic regression showed that the risk factors related to HIV infection included Ruili City,inpatient,being male,ethnic Jinbo and ethnic Dai,aged from 19 to 30and from 31 to 50years old,and being illiterate.Conclusion PITC in Dehong prefecture played an important role in detecting HIV cases and should be promoted in those towns with serious HIV epidemic.
