妊娠期糖尿病血糖控制水平对母婴妊娠结局的影响

Effect of blood glycemic control level of patients with gestational diabetes mellitus on maternal and infantile outcomes

目的探讨妊娠期糖尿病(GDM)患者血糖控制水平对母婴结局的影响。方法选择济宁市传染病医院医院2009年1月-2011年12月收治的102例GDM患者,并据血糖控制水平将其分为血糖控制满意组和血糖控制不满意组,对其母婴的结局进行比较分析。结果 GDM患者早产儿、巨大儿、新生儿窒息、羊水过多、剖宫产率及妊娠期高血压疾病发生率均高于正常孕妇(均P<0.05)。血糖控制满意组早产儿、巨大儿、新生儿窒息、羊水过多及妊娠高血压疾病发生率均低于血糖控制不满意组(均P<0.05),但剖宫产发生率比较,差异均无统计学意义(均P>0.05)。结论临床上应加强系统的健康宣教,重视早期血糖检测,一旦确诊为GDM,应积极进行运动、饮食、胰岛素疗法等综合治疗控制血糖水平,可提高围生儿及产妇的生存质量。

Objective To explore the effect of blood glycemic control level of patients with gestational diabetes mellitus （GDM） on ma- ternal and infantile outcomes. Methods A total of 102 GDM patients treated in the hospital from Jannm7 2009 to December 2012 were se- lected and divided into blood glucose control satisfaction group and blood glucose control dissatisfaction group according to blood glucose con- trol level; maternal and infantile outcomes in the two groups were compared and analyzed. Results The incidence rates of premature birth, macrosomia, neonatal asphyxia, hydramnios, cesarean section, and hypertensive disorder complicating pregnancy （ttDCP） in GDM women were statistically significantly higher than those in normal pregnant women （P〈0. 05 ） . The incidence rates of premature birth, macr0somia, neonatal asphyxia, hydramnios, and HDCP in blood glucose control satisfaction group were statistically significantly lower than those in blood glucose control dissatisfaction group （P〈0. 05 ）, but there was no statistically significant difference in the incidence rate of cesarean deli~,ery between the two groups （P〉0.05） . Conclusion Systematic health education should be strengthened, more attention should be paid to early blood glucose test; comprehensive treatment （exercise, diet, insulin therapy） should be taken to control blood glucose and improve perinatal and maternal survival quality.
