基于社区的2型糖尿病合并抑郁现况研究

Community based cross-sectional survey on patients with Type 2 diabetes complicated with depression

目的:研究社区2型糖尿病患者(T2DM)抑郁情绪患病情况以及影响抑郁患病的危险因素。对象与方法:采用分层随机抽样的方法调查成都市社区卫生中心随访的T2DM 714人,采用自编调查问卷及SDS量表进行调查。问卷使用epidata3.0录入数据库,采用SPSS17.0统计软件进行统计分析。结果:T2DM患者抑郁症状患病率43.32%,其中轻度抑郁患病率19.74%,中度抑郁患病率19.18%,重度抑郁患病率4.40%,男女间无统计学差异(χ2=6.335,P=0.096)。以标准分为因变量,将可能的抑郁症状危险因素纳入回归模型,采用Stepwise法,α=0.05,β=0.10进行逐步回归,结果筛选出5个影响抑郁症状的危险因素,分别是出现糖尿病并发症(P=0.002)、退休/无业(P=0.008)、目前感觉经济压力大(P=0.003)、丧偶(P=0.025)、文化程度低(P=0.040)。结论:T2DM患者伴发抑郁患病率高,T2DM中出现并发症、文化程度低、丧偶、经济压力大的患者易伴发抑郁情绪,是心理干预工作的重点人群。

Objective To investigate the prevalence of depression in patients with Type 2 diabetes（ T2DM）,and analyze its risk factors.Methods 714 cases of patients with T2 DM were selected by multi- stage stratified random sampling from community health service centers of Chengdu,and surveyed with self-made questionnaire and SDS scale.Data was input by epidata3.0 database and analyzed with software of SPSS17. 0. Results The prevalence of depression in patients with T2 DM was 43. 32%,in which the prevalence of mild depression was 19.74%,of moderate depression was 19.18% and major depression was 4.40%.There had no statistic difference between male and female（ P = 0.096）.Multiple regression model showed that there had 5 risk factors of depression,which were diabetes complications（ P = 0.002）,retires / unemployed（ P = 0.008）,feeling heavy economic pressure（ P = 0. 003）,death of spouse（ P = 0. 025） and lower education level（ P = 0. 040）. Conclusion The prevalence of depression in patients with T2 DM was high.The patients with diabetes complications,low education level,death of spouse and feeling heavy economic pressure were groups in higher risk of depression,which need to be psychologically intervened.
