臭氧联合桃红四物汤治疗老年膝骨性关节炎临床观察

The Effect of Ozone Combined with Taohongsiwu Decoction on Treatment of Senile Patients with Knee Osteoarthritis

目的探讨臭氧联合桃红四物汤治疗老年膝骨性关节炎(KOA)的疗效。方法选取2013年5月至2014年5月广州医学院荔湾医院收治的98例KOA患者,采用随机数字表法将患者分为联合治疗组(n=48)和臭氧治疗组(n=48),其中臭氧治疗组仅给予臭氧治疗,每次注射15 m L,每周1次;联合治疗组在臭氧治疗组的基础上加服桃红四物汤治疗,每日1剂。比较两组患者治疗前、治疗后1个月、2个月、3个月时的视觉模拟评分(VAS)、膝关节功能评分、血清白细胞介素6(IL-6)的水平以及治疗3个月后两组患者的疗效。结果两组KOA患者的VAS、膝关节功能评分以及血清IL-6的水平在组间、不同时点间以及组间·不同时点间比较差异均有统计学意义(P<0.05),随着治疗时间的延长膝关节功能评分逐渐升高,且联合组高于臭氧治疗组,而VAS和IL-6随着治疗时间的延长而降低,且联合组低于臭氧治疗组。联合治疗组的疗效显著高于臭氧治疗组(Z=2.286,P<0.05)。结论臭氧联合桃红四物汤治疗老年KOA可显著减轻患者疼痛,改善膝关节功能,提高临床疗效,值得临床推广应用。

Objective To evaluate the curative effect of ozone combined with Taohongsiwu decoction in treatment of senile patients with knee osteoarthritis（KOA）. Methods Total of 98 senile patients with KOA hospitalized in Liwan Hospital of Guangzhou Medical College from May 2013 to May 2014 were divided into combined treatment group （n = 48 ） and ozone treatment group （n = 48 ） according to random number table method. The ozone treatment group was treated with ozone, 15 mL every time, one time every week. Ozone combined with Taohongsiwu decoction was used in combined treatment group, 1 time/d. VAS score, Lysholm knee functional scores and serum IL-6 level before treatment, 1 month, 2 months and 3 months after treatment were compared, the clinical efficacy 3 months after treatment were compared too. Results VAS, Lysholm knee functional scores and serum IL-6 levels between different groups, different time points and group different time points showed statistically significant difference （ P 〈 0.05 ）, as treatment extended, the Lysholm knee functional scores gradually increased, and the combined treatment group was higher than the ozone treatment group, while the VAS and IL-6 decreased, and the combined treatment group was lower than the ozone treatment group. The clinical efficacy of the combined treatment group was statistically higher than the ozone treatment group （ Z = 2. 286, P 〈 0.05 ）. Conclusion Ozone combined with Taohongsiwu decoction treatment on senile KOA patients has significant curative effect, thus is worthy of clinical application.
