中西医结合治疗支气管哮喘寒哮证临床观察

Clinical Observation on Integrated Chinese-western Therapy in Treatment of Cold Bornehial Asthma

目的观察中西医结合治疗支气管哮喘寒哮证的临床疗效。方法 80例支气管哮喘寒哮证患者随机分为两组,每组40例。对照组以阿奇霉素静脉滴注和布地奈德混悬液雾化吸入治疗,治疗组在对照组治疗基础上给予小青龙汤治疗,2周后观察肺功能指标肺活量(FVC)、呼气的峰值流速(PEF)、第1秒呼气容积(FEV1),C反应蛋白(CRP)、白细胞介素-5(IL-5)、症状积分及临床疗效。结果治疗组总有效率明显优于对照组(P<0.05);两组治疗后FVC、PEF、FEV1明显增加,日间和夜间症状积分明显降低,CRP和IL-5明显降低,和治疗前比较差异有统计学意义(P<0.05,P<0.01);治疗组在改善FVC、PEF、FEV1、日间和夜间症状积分方面优于对照组(P<0.05)。结论阿奇霉素静脉滴注和布地奈德混悬液雾化吸入治疗基础上配合小青龙汤治疗支气管哮喘寒哮证,在改善肺功能和症状积分方面具有良好的效果。

Objective To investigate the clinical effect of integrated Chinese- western therapy on cold bornehial asthma. Methods 80 cases of cold bronchial asthma patients were randomly divided into 2 groups,40 cases in each group. The control group received azithromycin intravenous drip and budesonide mixed suspension liquid atomization inhalation,the treatment group received Xiaoqinglong decoction on the basis of the control group. After 2 weeks of treatment,lung function index of vital capacity（ FVC）,expiratory peak expiratory flow（ PEF）,one second forced expiratory volume（ FEV1）,C reactive protein,IL- 5,symptom score and clinical curative effect were evaluated before and after treatment. Results FVC,PEF,FEV1 increased significantly after treatment in two groups,daytime and nighttime symptom score significantly decreased,C reactive protein and IL- 5 significantly reduced,there were significant differences between pretreatment and posttreatment（ P〈0. 05,P〈0. 01）,there were significant differences between control group and treatment group in improving the FVC,PEF,FEV1,daytime and nighttime symptom scores（ P〈0. 05）,treatment group was significantly higher than control group（ P〈0. 05）. Conclusion There is a good effect in improving lung function and symptom score by mixed intravenous infusion of azithromycin and budesonide suspension inhalation therapy with Xiaoqinglong decoction in treatment of cold bronchial asthma,is worth popularizing in clinical use.
