带线锚钉联合克氏针内固定治疗尺骨鹰嘴粉碎性骨折的效果观察

Effect observation of line anchor combined with internal fixation with Kirschner in treatment of comminuted olecranon fractures

目的探讨带线锚钉联合克氏针内固定治疗尺骨鹰嘴粉碎性骨折的临床效果。方法选择2013年1月~2014年6月我院收治的80例尺骨鹰嘴粉碎性骨折患者,按照随机数字表法将其分为观察组和对照组,各40例。观察组采用带线锚钉联合克氏针内固定治疗,对照组采用克氏针内固定治疗,比较两组术后肘关节活动度及并发症发生情况,并统计治疗后两组的肘关节Mayo功能评分。结果观察组肘关节屈曲度、旋前角度及旋后角度均显著大于对照组（P〈0.05）,伸直度丢失情况小于对照组（P〈0.05）。观察组切口感染、内固定松动、肘关节骨化性肌炎及皮肤溃破窦道形成的发生率低于对照组（P〈0.05）。观察组肘关节Mayo功能评分优秀率高于对照组（P〈0.05）。结论带线锚钉联合克氏针内固定治疗尺骨鹰嘴粉碎性骨折,能有效改善肘关节活动度,减少手术相关并发症,促进肘关节功能恢复。

Objective To investigate the clinical effect of line anchor combined with internal fixation with Kirschner in treatment of comminuted olecranon fractures. Methods 80 patients with comminuted olecranon fractures admitted into our hospital from January 2013 to June 2014 were selected.According to the random number table method,they were divided into observation group and control group,with 40 cases in each group.The observation group was equipped with line anchor combined with internal fixation with Kirschner,the control group was used Kirschner.The mobility of elbow joint and complications occurred in two group was compared,and the Mayo elbow score was calculated after treatment.Results The flexion of elbow joint,pronation and supination angle angles in observation group were greater than the control group（P〈0.05）,straightness loss was less than the control group（P〈0.05）.The incidence rate of wound infection,internal fixation loosening,elbow myositis ossificans skin ulceration and sinus formation ratio in observation group were lower than those of control group（P〈0.05）.The excellent rate of Mayo elbow score in observation group was higher than that of control group（P〈0.05）. Conclusion Line anchor combined with internal fixation with Kirschner can effectively improve mobility in treatment of comminuted olecranon fractures,reduce surgical complications,promote the elbow joint function recovery.
