醒脑静对脑出血急性期神经功能的改善作用及对血清Tf与Ft的影响 被引量:35

摘要 目的:探讨醒脑静对脑出血急性期患者血清转铁蛋白(Tf)及铁蛋白(Ft)的变化,并观察神经功能改善状况,为评估脑出血急性期治疗提供依据。方法:选择2004年2月—2010年2月医院急性脑出血患者122例,随机分为对照组和治疗组,对照组60例,进行常规治疗,治疗组62例,在对照组基础上应用醒脑静治疗,选取同期健康体检者30例,为正常组。入院后第1、3、7、14、21天检测血清Tf及Ft水平。采用美国国立卫生院神经功能缺损评分标准(NIHSS)在入院第1、7、14、21天对患者进行神经功能缺损程度评分。结果:治疗组和对照组Tf在第1天高于正常组(P1=0.025,P2=0.031);对照组Tf在发病第3天高于正常组(P1=0.014)。治疗组Tf在第3天较对照组显著下降(P=0.034)。治疗组和对照组血清Ft在发病后第7天、14天与正常组比较,差异具有统计学意义(P1=0.018,0.025,0.001,0.038)。对照组Ft在第21天高于正常组(P=0.005);治疗组第7天、第14天血清Ft低于对照组(P1=0.009,P2=0.038)。血量在10~20 m L及≥20 m L时,治疗组血清Tf和Ft均低于对照组,差异具有统计学意义(P1=0.006,0.007;0.021,0.039)。对照组和治疗组第7天、14天、21天NIHSS评分比较,差异有统计学意义(P=0.004,0.002,0.001)。结论:醒脑静注射液可以降低脑出血急性期血清转铁蛋白及铁蛋白水平,改善患者神经缺损程度。 Objective: To explore the serum transferrin( Tf) and ferritin( Ft) changes of Xingnaojing Injection on acute cerebral hemorrhage( ICH) patients with intracerebral hemorrhage. And to observe the improvement of neurological function and provide the basis for the treatment of acute stage of assessment of cerebral hemorrhage. Methods: 122 acute cerebral hemorrhage patients were selected in the hospital from February 2004 to February 2010 and were randomly divided into the control group and the treatment group. 60 cases of the control group used conventional treatment and 62 cases of the treatment group added Xingnaojing Injection based on the control group's treatment. 30 cases were selected as normal control group of healthy people. The serum Tf and Ft levels were detected after 1,3,7,14 and 21 days. America NIH neural function defect score standard( NIHSS) was used for the degree of neurological deficit score of patients in hospitalization after 1,7,14 and 21 days.Results: Tf of the treatment group and the control group on the first day was higher than that of the normal group( P1= 0.025,P2= 0.031). Tf of the control group on the third day was higher than that of the normal group( P1= 0.014). Tf of the treatment group was lower than that of control group on third day( P = 0.034). The serum Ft of the treatment group and the control group after 7 and 14 days had statistical significance compared with those of the normal group( P1= 0.018,0. 025,0. 001,0. 038). Ft of the control group in 21 days had significance compared with that of the normal group( P = 0.005). The serum Ft of the treatment group of 7 and 14 days waslower than those of the control group( P1= 0.009,P2= 0.038). When bleeding volume was 10 ~ 20 m L and ≥20 m L,the serum Tf and Ft were lower than those of the control group and the difference was statistically significant( P1= 0. 006,0. 007; 0. 021,0. 039).NIHSS scores of the treatment group and the control group of 7,14 and 21 days were statistically significant( P = 0.004,0.002,0.001).Conclusion: Xingnaojing may reduce levels of serum transferrin( Tf) and ferritin( Ft) in patients with intracerebral hemorrhage( ICH) at acute stage. It improves the degree of neurological deficit.
