血必净注射液治疗老年慢性阻塞性肺病急性期

Clinical Observation of Xuebijing Injection on Elderly Patients with Chronic Obstructive Pulmonary Disease in Acute Exacerbation

目的:观察血必净注射液治疗老年慢性阻塞性肺病(COPD)患者急性期疗效的临床观察。方法:选取COPD急性加重患者82例,随机分为两组。在对症支持治疗基础上,对照组予头孢哌酮钠舒巴坦钠1.5 g,5%葡萄糖注射液100 m L稀释后静脉滴注,bid;观察组在对照组治疗的基础上加用血必净注射液50 m L加生理盐水100 m L,静脉滴注,bid。疗程14 d,比较两组治疗前后患者病情积分、肺功能及炎性因子水平改善情况及临床疗效。结果:治疗后,观察组病情积分,第一秒用力呼气量(FEV1),FEV1/用力肺活量(FVC),C-反应蛋白(CRP)及降钙素原(PCT)水平均较对照组明显改善(P<0.05);治疗组有效率为90.24%,高于对照组的80.49%(P<0.05)。结论:老年COPD患者急性加重期联合使用血必净注射液治疗能够显著改善临床症状及体征,提高肺功能水平,降低体内炎性反应,提高临床疗效。

Objective： To observe the clinical effect of Xuebijing injection on elderly patients with chronic obstructive pulmonary disease（ COPD） in acute exacerbation. Method： The 82 cases of elderly patients with COPD in acute exacerbation were randomly divided into two groups. Based on symptomatic and supportive treatments,the patients in control group were treated with 1. 5 g cefoperazone sodium and sulbactam sodium which was diluted by 100 m L 5% glucose injection for intravenous drip,bid. Based on the treatment of control group,the patients in observation group also received 50 m L Xuebijing injection plus 100 m L normal saline,injection,by intravenous drip,bid. The treatment course was 14 days for both groups. The disease severity score,pulmonary function and the level of inflammatory factor before and after treatment were compared between two groups. Result：Compared with the control group,the total disease severity scores,level of forced expiratory volume in first second（ FEV1）,FEV1/ forced vital capacity（ FVC）,C-reactive protein（ CRP） and calcitonin（ PCT） of the observation group were obviously improved after the treatment（ P〈0. 05）. The effective rate of the treatment group was90. 24%,higher than 80. 49% in control group（ P〈0. 05）. Conclusion： For elderly patients with chronic obstructive pulmonary disease in acute exacerbation, Xuebijing injection can significantly improve clinical symptoms,signs and the level of pulmonary function,reduce inflammatory reaction,and improve the clinical efficacy.
