内固定术与人工髋关节置换术对老年股骨粗隆间骨折疼痛及术后髋关节活动度的影响

Effects of iternal fixation and hip replacement on postoperative pain and range of motion of hip joint in the treatment of intertrochanteric fractures in the elderly

目的:探讨内固定术和人工髋关节置换术治疗老年股骨粗隆间骨折的临床效果。方法:选择股骨粗隆间骨折患者117例,接受动力髋螺钉(DHS)内固定术治疗61例,接受人工髋关节置换术治疗56例,比较两组的相关指标变化。结果:与内固定术组比较,人工髋关节置换术能显著降低手术时间、术中出血量、术后卧床时间以及并发症发生率,而且患者的疼痛缓解和髋关节功能恢复更好(P<0.05)。结论:使用人工髋关节置换术治疗老年股骨粗隆间骨折,临床疗效更加显著,患者的疼痛缓解及髋关节活动度恢复更好。

Objective： To compare the efficacy of internal fixation（ DHS） and hip replacement in the treatment of intertrochanteric fractures in the elderly. Methods： All 117 patients with intertrochanteric fractures in our hospital were analysed. 61 patients were treated by internal fixation（ DHS） and 56 patients were treated by hip replacement. And compared the correlation index of two groups. Results： The operation time,blood loss,postoperative bed time and postoperative complications of hip replacement group were lower than DHS group. And postoperative pains were improved and hip joint functions were better（ P 0. 05）. Conclusion： It is a more effective method to treat the intertrochanteric fractures in the elderly with hip replacement. It can relieve postoperative pain and improve range of motion of hip joint in the elderly patients.
