标准护理流程下结肠造口底盘更换时间的研究

Changing frequency of the wafer of colostomy bags under standard nursing procedure

目的探讨标准护理流程下结肠造口底盘更换的适宜时间。方法将156例结肠造口患者采用随机数字表法分为A、B、C三组,A、B组各53例,C组50例,三组均采用标准护理流程进行造口护理,分别间隔2-3d、4-5d、6-7d更换1次造口底盘,观察并比较造口周围皮肤状况、造口底盘浸渍宽度等。结果三组造口底盘浸渍宽度、造口周围皮肤并发症发生率及造口底盘有效更换次数比较,差异有统计学意义（P〈0.05,P〈0.01）,A、B组效果优于C组（均P〈0.05）。结论结肠造口患者造口底盘在标准护理流程下,以间隔4-5d更换1次为宜,能减少造口周围皮肤并发症的发生,减轻患者痛苦和经济负担。

Objective To investigate the changing frequency of the wafer of colostomy bags under standard nursing procedure.Methods Totally 156 colostomy patients were randomly divided into group A（n=53）,group B（n=53）and group C（n=50）according to a random number table.The 3groups were cared according to standardized nursing procedure,and the wafer of their colostomy bags were replaced at an interval of two to three days,four to five days and six to seven days respectively.Observation and comparison were conducted on the condition of the skin around the periosteal wound,the width of liquid seeps underneath the wafer around the stoma,and so on.Results There were significant differences in the width of liquid seeps underneath the wafer,skin complications around the stoma,and the counts of changing the wafer among the 3groups（P〈0.05,P〈0.01）,with group A and group B better than group C（P〈0.05 for both）.Conclusion Under standard nursing procedure,it is recommended to change the wafer of colostomy bags at an interval of four to five days,which can reduce skin complications around stoma,and minimize patients′suffering and economic burden.
