摘要 目的比较CT征象和MRI在原发骨肉瘤诊断中应用价值,提高原发性骨肉瘤的影像学诊断的准确性。方法回顾分析60例经病理检查证实的原发性骨肉瘤的CT或MRI检查资料。结果骨肉瘤的基本影像表现为骨质破坏、瘤骨、骨膜反应和软组织肿块。MRI对骨肉瘤的正确诊断率为100.00%,CT的正确诊断率为95.65%。CT对细骨质破坏、瘤骨的显示敏感;MRI能更清楚地显示跳跃性的肿瘤组织,周围软组织的浸润范围,且能清楚显示瘤周水肿带。CT及MRI对Codman三角的检出率均较低。29例MRI表现T1WI表现以不均匀低信号为主,T2WI均呈混杂信号。结论CT、MRI是X线的补充手段,在骨肉瘤的早期诊断、侵袭范围的确定具有明显优势,对临床治疗方案的选择的指导意义。 Objective To study the application value of CT signs and MRI in the diagnosis of primary osteosarcoma,and improve the accuracy of imaging diagnosis to primary osteosarcoma. Methods Retrospective analysis of the CT or MRI data of 60 cases of patients confirmed to be primary osteosarcoma by pathology were conducted. Results The basic imaging findings of primary osteosarcoma were bone destruction,tumor bone,periosteal reaction and soft tissue mass. The osteosarcoma correct diagnostic rate of MRI was 100. 00%,while the CT was 95. 65%. CT showed sensitively for bone destruction and tumor bone. MRI could show more clearly the skip tumor tissues and infiltration range of surrounding soft tissues,and could clearly see the edema zone surrounding tumor. The detection rates of Codman triangle by CT and MRI were low. Among 29 cases of MRI,the performance of T1 WI given priority to the low density of homogeneous signal,while all were the mixed signal in the T2 WI. Conclusion As supplements to the X-ray,the CT and MRI has obvious advantages in the early diagnosis of osteosarcoma and the determination of the attacked scope. It is of guiding significance to the choice of clinical treatment.
