P53在纯型卵巢内胚窦瘤中表达及其临床意义

Expression of P53 in pure ovarian yolk sac tumors and its clinical significance

目的分析P53在卵巢内胚窦瘤的表达及临床意义。方法回顾性分析辽宁省肿瘤医院1984-2007年收治的64例纯型卵巢内胚窦瘤患者的临床病理资料,免疫组织化学法检测P53在卵巢内胚窦瘤中的表达,分析各因素对预后的影响。结果以铂类为基础的化疗方案,化疗疗程数>3个、残余瘤直径≤2 cm、腹水量≤100 ml是影响预后的独立因素。P53并不是影响预后的独立因素。结论以铂类为基础的化疗方案,化疗疗程数、残余瘤直径和腹水量是影响预后的独立因素。

Objective To investigate the P53 expression in ovarian germ sinus tumor and clinical significance. Methods We performed a review of 64 patients with pure ovarian YST from our department between 1984 and 2007,and the expression of P53 was investigated by immunochemistry. Results Patients with stage Ⅰ disease had a more favorable prognosis than those with stage Ⅲ and Ⅳ disease（ P〈0. 01）. Cisplatin based chemotherapy regimens gave better results than those without cisplatin（ P〈0. 05）. The difference in prognosis was significant in cases in which the size of residual tumor was 2 cm in diameter（ P〈0. 01）,in cases in which chemotherapy courses 3（ P〈0. 05） and in cases in which ascites was either absent or 100 ml in volume（ P〈0. 05）,P53 status had no significant correlation with the prognosis in this study. Conclusion Ascites volume,chemotherapy courses and tumor-reductive surgery strongly affected the prognosis of this disease. Cisplatin based chemotherapy after surgery was superior to chemotherapy without cisplatin.
