摘要 目的:研究补体C1q肿瘤坏死因子相关蛋白3(CTRP-3)在肥胖患者的表达及意义。方法:选择肥胖患者402例(肥胖组)和正常体检人员405例(正常对照组)。分析CTRP-3水平与有关指标的相关性,并利用多因素回归分析研究肥胖的独立危险因素。结果:与正常对照组相比,肥胖组人体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、空腹血糖(FBG)、胰岛素、稳态模型胰岛素抵抗指数(HOMA-IR)、糖化血红蛋白(HbA1c)、高敏C反应蛋白(hsCRP)水平均显著升高,而高密度脂蛋白胆固醇(HDL-C)、脂联素(APN)、瘦素和CTRP-3水平均显著降低,P〈0.05或〈0.01。Spearman相关分析显示,校正年龄、性别后,CTRP-3水平与BMI(r=-0.221)、SBP(r=-0.031)、DBP(r=-0.043)、TC(r=-0.147)、LDL-C(r=-0.051)、TG(r=-0.743)、FBG(r=-0.238)、胰岛素(r=-0.053)、HOMA-IR(r=-0.281)、HbA1c(r=-0.741)和hsCRP水平(r=-0.216)呈显著负相关(P〈0.05或〈0.01),与HDL-C(r=0.351)、APN(r=0.852)和瘦素水平(r=0.641)呈显著正相关,P均〈0.05。多因素回归分析显示,在调整了其他影响因素后,与高三分位数CTRP-3组相比,中三分位数CTRP-3组OR值6.47(95%CI 3.58~12.18)和低三分位数CTRP-3组OR值12.39(95%CI 3.58~29.15)显著升高,P均〈0.01。结论:CTRP-3水平与肥胖相关因素显著相关,低CTRP-3水平是肥胖症的独立危险因素。 Objeettive: To study expression of complement Clq tumor necrosis factor related protein 3 (CTRP-3) in obese patients and its significance. Methods: A total of 402 obese patients were enrolled as obesity group and 405 normal people undergoing physical examination were regarded as normal control group. The correlation among CTRP-3 level and related indexes were analyzed, and multi- factor regression analysis was used to study independent risk factors for obesity. Results: Compared with normal control group, there were significant rise in body mass index (BMI), homeostasis model - insulin resistance index (HOMA- IR), levels of systolic blood pressure (SBP), diastolic blood pressure (DBP), total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), triglyceride (TG), fasting blood glucose (FBG), insulin, glycosylated hemoglobin (HbAlc) and high sensitive C reactive protein (hsCRP), and significant reductions in levels of high density lipoprotein cholesterol (HI)L-C), adiponectin (APN), leptin and CTRP-3 in obesity group, P〈0.05 or 〈0. 01. Spearman correlation analysis indicated that after age and gender correction, CTRP-3 level was significant inversely correlated with BMI (r = - 0. 221), SBP (r = - 0. 031), DBP (r = - 0. 043), TC ( r = - 0. 147), LDL-C (r= -0.051), TG(r= -0.743), FBG (r= -0.238), insulin (r= -0.053), HOMA-IR (r= -0.281), HbAlc ( r = - 0. 741) and hsCRP levels ( r = - 0. 216), P〈0. 05 or 〈0.01, and significant positively correlated with lev- els of HDL-C (r = 0. 351), APN (r = 0. 852) and leptin (r = 0. 641), P〈0. 05 all. Multi- factor regression analysis indi- cated that after correcting other influencing factors, compared with high tertile CTRP-3 group, there were significant rise in OR value in middle tertile CTRP-3 group (OR= 6.47, 95%CI 3.58 - 12.18) and low tertile CTRP-3 group (OR= 12.39, 95%CI 3.58 - 29.15), P〈0. 01 all. Conclusion: CTRP-3 level is significantly correlated with obesity- related fac- tors, and it's an independent risk factor for obesity.
