大椎穴施以三重法防治变应性鼻炎及对血清SP、IL-4的影响

Dazhui(GV14) with Three Intensified Method in Preventing and Treating Allergic Rhinitis and Its Effect on Serum SP and IL-4

目的观察针刺联合大椎穴施以三重法防治变应性鼻炎的临床疗效及对血清SP、IL-4的影响。方法将80例变应性鼻炎患者随机分为针刺联合三重法组(三重法组)、针刺联合西药组(针药组),每组40例。三重法组在针刺基础上,大椎穴施重刺络、重拔罐、重艾灸之三重法;针药组在针刺基础上,口服氯雷他定片(开瑞坦)10 mg。观察治疗1个月及治疗6个月后两组患者症状与体征积分变化、临床疗效及治疗前后血清SP、IL-4的水平,并进行比较。结果针刺联合三重法组总有效率为92.5%,明显优于针药组65.0%(P<0.05);两组患者治疗后1个月、6个月后症状与体征积分均有所下降(P<0.01,P<0.05),三重法组较针药组改善更为显著(P<0.05);两组患者治疗后血清SP、IL-4含量较治疗前均有下降(P<0.01,P<0.05)。结论针刺联合大椎穴施以三重法治疗变应性鼻炎临床疗效肯定,且远、近期疗效均明显优于针刺加口服西药治疗。其作用机理可能与降低血清SP、IL-4的含量有关。

Objective To observe the clinical efficacy of acupuncture plus three intensified method at Dazhui（GV14） in preventing and treating allergic rhinitis, as well as its effect on serum substance P（SP） and interleukin（IL）-4. Method Eighty patients with allergic rhinitis were randomized into a group with acupuncture plus three intensified method（three intensified group） and a group with acupuncture plus Western medication（acupuncture-medication group）, 40 cases in each group. For the three intensified group, in addition to acupuncture treatment, the three intensified method at Dazhui, including intensified collateral-pricking, intensified cupping, and intensified moxibustion, was also applied; the acupuncture-medication group was by acupuncture plus orally taking 10 mg Loratadin tablet. Respectively after 1-month and 6-month treatments, the symptom and body sign score, clinical efficacy, serum SP, and IL-4 levels were observed and compared. Result The total effective rate was 92.5% in the three intensified group, significantly higher than 65.0% in the acupuncture-medication group（P〈0.05）; the symptom and body sign scores dropped significantly in both groups after 1-month and 6-month treatment compared to the pre-treatment results（P〈0.01, P〈0.05）, while the improvement in the three intensified group was more significant（P〈0.05）; the serum SP and IL-4 contents dropped significantly in both groups compared to the pre-treatment results（P〈0.01,P〈0.05）. Conclusion Acupuncture plus three intensified method at Dazhui is effective in treating allergic rhinitis, and its long-term and short-term efficacies are both more significant than acupuncture plus orally taking Western medication. Its action mechanism is plausibly related to the decrease of serum SP and IL-4 contents.
