Toll样受体7在系统性红斑狼疮合并高危人乳头瘤病毒感染中的作用 被引量:6

摘要 目的观察Toll样受体7(TLR7)在系统性红斑狼疮(SLE)、人乳头瘤病毒(HPV)、SLE+HPV感染者宫颈上皮细胞及不同HPV亚型宫颈癌细胞株中的表达,并探讨其意义。方法选择行子宫颈液基细胞学检查者20例,其中,SLE合并HPV DNA阳性5例(SLE&HPV+组)、SLE合并HPV DNA阴性5例(SLE&HPV-组)、正常合并HPV DNA阳性5例(CTL&HPV+组)、正常合并HPV DNA阴性5例(CTL&HPV-组)。收集的宫颈脱落细胞一份用于宫颈液基细胞学检查,剩余的标本用于高危型HPV及低危型HPV检测,另一份用于流式鉴定及胞内TLR7检测。结果流式cytokeratin+CD45-设门鉴定的上皮细胞内TLR7表达的平均荧光强度:SLE&HPV+组<SLE&HPV-组<CTL&HPV+组<CTL&HPV-组。不同HPV亚型人宫颈癌细胞系Caski(HPV16阳性)、He La细胞(HPV18阳性)及HPV阴性的C33A细胞系内TLR7表达的平均荧光强度:Caski<He La<C33A。SLE&HPV+组SLE疾病活动性评分显著高于SLE&HPV-组,且更容易合并器官损伤;且病情复发指数也明显高于SLE&HPV-组;同样,SLE&HPV+组患者全部为Anti-ds DNA阳性,而SLE&HPV-组仅占20%。结论 TLR7下调可能与HPV逃避宿主免疫防御从而导致病情长期潜伏相关;HPV可能通过核酸特异性的TLR7激活固有免疫和调节免疫系统,导致病情发作或者非活动性SLE的复发;多种因素包括病毒感染本身、药物因素以及病情活动复发均可致SLE患者体内HPV持续易感。
