丹参酮IIA磺酸钠注射液对过敏性紫癜患者紫癜消退及肾受累的影响

Effect of sodium tanshinone IIA sulfonate injection on purpura subsiding and renal involvement in patients with allergic purpura

目的探讨采用丹参酮IIA磺酸钠注射液治疗过敏性紫癜对患者紫癜消退及肾受累的影响。方法选取2013年8月至2014年10月已确诊为过敏性紫癜的患者68例,每组34例。采用随机数字表将患者分为实验组和对照组,每组34例。对照组给予常规治疗,实验组在常规治疗的基础上加静脉滴注丹参酮IIA磺酸钠注射液。观察两组过敏性紫癜患者紫癜消退情况及肾脏受累情况。结果入院时两组各有6例为肾型紫癜,基础肾受累率相当(P>0.05);入院后新发肾脏受累实验组2例(5.88%),对照组9例(26.47%),新发肾脏受累率实验组明显低于对照组,差异有统计学意义(P<0.05)。且治疗后实验组原6例肾型紫癜患者肾脏损害减轻,逐渐转为镜下血尿;而对照组原6例肾型紫癜的肾脏损害几乎无改善。结论在常规治疗基础上加用丹参酮IIA磺酸钠注射液治疗过敏性紫癜,可降低患者的肾脏受累率,改善肾脏损害程度。

Objective To investigate the effects of sodium tanshinone IIA sulfonate injection on purpura subsiding and renal involvement in patients with allergic purpura.Methods A total of 68 patients with confirmed allergic purpura between August 2013 and October 2014 were enrolled in this study.According to random number table method,the patients were divided into two groups（n = 34 each）：experiment group and control group.The conventional treatment was given in control group,and the intravenous infusion of sodium tanshinone IIA sulfonate was administered based on the routine treatment in experiment group.The condition of purpura subsiding and the degree of renal involvement were compared between two groups.Results There were no significant differences in cure rate of purpura subsiding（44.12% vs 38.24%,P 0.05）and total effective rate（76.47% vs 79.41%,P 0.05）between two groups.There were six patients with type kidney purpura on admission in each group,so the basal renal involvement rates were equal in two groups.The renal involvement of new occurrence were two cases（5.88%）in experiment group,nine cases（26.47%）in control group,and the incidence of renal involvement of new occurrence in experiment group significantly decreased compared with control group（P 0.05）.In original six patients with type kidney purpura in each gruop,the renal damage was alleviated and was gradually turned to microscopic hematuria after treatment in experiment group,while there was no obvious improvement in renal damage of control group after treatment.Conclusion The treatment of sodium tanshinone IIA sulfonate injection on the basis of routine therapy can decrease renal involvement rate and improve the degree of renal damage in allergic purpura patients.
