3.0T MRI扩散成像研究兔系膜增生性肾小球肾炎初探

Diffusion imaging using 3T MRI to evaluate Mesangial Proliferative Glomerulonephritis: an experimental study in rabbits

摘要 目的探讨3.0T MRI扩散成像评价兔系膜增生性肾小球肾炎(mesangial proliferative glomerulonephritis,Ms PGN)的可行性。方法实验兔共24只,随机分为2组,对照组8只,模型组16只,造模后4周及8周分别对实验兔行3.0T MRI检查。同一时间点皮髓质ADC值(表观扩散系数Apparent Diffusion Coefficient)、FA值均数比较应用t检验。不同分组间皮、髓质ADC值、FA值(各向异性分数Fractional Anisotropy)均数的比较采用单因素方差分析。结果实验动物各个时间点皮质ADC值均大于髓质,两者组内比较对照组、模型组4周差异有统计学意义。模型组4周、模型组8周皮质与髓质的ADC均值与对照组相比逐渐降低,对照组皮质ADC值与模型组8周皮质ADC值、模型组4周皮质ADC值与模型组8周皮质ADC比较,差异均有统计学意义。对照组髓质ADC值与模型组8周髓质ADC值比较,差异有统计学意义。实验动物各个时间点皮质FA值均小于髓质,三组组内比较差异均有统计学意义。三个时间点组间比较皮质FA值差异均有统计学意义,而髓质FA值三组间比较差异无统计学意义。结论 3.0T MRI弥散成像能显示兔Ms PGN肾脏的皮髓质ADC值及FA值变化情况,且皮质ADC值、FA值较髓质改变更为显著。 Objective To investigate the feasibility of MR diffusion image in evaluating rabbit Mesangial Proliferative Glomerulonephritis. Methods 16 rabbits with Ms PGN and 8 normal rabbits( group A) were performed conventional MR and DWI f MRI. The rabbits with Ms PGN were divided into 2 groups,4 weeks( group B) and 8 weeks( group C) after model,respectively.Independent-Samples T Test was used to compare mean ADC value and FA value between cortex and medulla in different groups.One-Way ANOVA( analysis of variance) was used to compare mean cortical ADC value and FA value,and medullary ADC value and FA value among three different groups. Results In three groups,the cortical ADC value was higher than medullary. There were statistical differences in group A and group B. ADC values of cortex and medulla of group B and group C were gradually decreased compared to group A. There were statistical differences between group A and group C in cortical ADC value,as well as group B and group C. There were statistical differences between group A and group C in medullary ADC value. The cortical FA value was lower than medullary. There were statistical differences in group A,group B and group C. There were statistical differences in cortical FA value between three groups. Conclusion 3. 0 T MRI DWI can display cortex and medulla ADC value and FA value changes of rabbit kidney with Ms PGN and cortical ADC values. FA values change more significant than the medulla.
