从全球视角看中国脑卒中疾病负担的严峻性

From the global views to understand the seriousness of the burden of stroke in China

目的分析对比国内外脑卒中疾病负担现状及趋势。方法以发病率、患病率、死亡率、伤残调整寿命年(DALY)等四个指标反映脑卒中疾病负担的严重程度。结果全球脑卒中的发病率有所降低,而中国的发病率仍然很高,远高于发展中国家水平;全球脑卒中的患病率稍有增高,中国脑卒中患病率也呈现上升趋势,并且城市高于农村,城市及农村脑卒中患病率均高于全球;全球脑卒中死亡率在下降,而中国脑卒中在上升,农村高于城市,农村和城市死亡率均高于全球水平;全球脑卒中导致的DALY下降,发展中国家的脑卒中DALY远高于发达国家,我国脑卒中导致的DALY仍然呈现上升趋势。结论我国目前脑卒中防控形势严峻;未来需要采取更多有效的预防与控制措施,建立标准化、规范化脑卒中数据库,定期及时进行大数据分析,为我国脑卒中的预防与控制提供更为可靠、详实的决策依据。

Objective To analyze and compare current situations and trends of stroke burden between domestic and oversea studies.Methods Incidence,prevalence,mortality and disability adjusted life year（ DALY） were used to reflect the severity of the burden of stroke. Results Compared with the findings from the various studies,the incidence of stroke had decreased globally,but the figure in China was still increasing,which was far more than that in those developing countries. The global prevalence of stroke had increased slightly,which was similar to that in China. It was obvious that urban prevalence was higher than that in rural area.Especially,both urban and rural prevalence were higher than that of global figure. Although the mortality of global stroke had reduced gradually,this number in China still showed an increasing curve. Rural mortality was higher than that in urban,and both were higher than the worldwide mortality. The global DALY caused by stroke had decreased; DALY in developing countries was much higher than that in developed countries. On the contrary,DALY caused by stroke in China showed an upward trend. Conclusion More effective measures are needed to prevent and control the severe status quo of stroke in China. Standard database of stroke should be established for epidemiological investigation and research in our country,and provide more evidences for stroke treatment and prevention.
