西甲硅油在结肠镜检查中的应用

The application of simethicone in colonoscopy

目的探讨结肠镜检查术前口服西甲硅油乳剂的祛泡作用。方法将2013年6月~2015年6月收治的1200例患者随机分成3组,均在结肠镜检查前6 h口服复方聚乙二醇电解质散。A组术前4 h口服西甲硅油乳剂,B组术前2 h口服西甲硅油乳剂,C组不服用西甲硅油乳剂,比较三组腔内气泡量以及病灶检出率。结果 A组A级355例,B级42例,C级3例;B组A级201例,B级197例,C级2例;C组A级88例,B级121例,C级105例,D级86例。A组肠腔内气泡量最少,C组最多,差异有统计学意义（P〈0.05）。A组病灶检出率最高（31.25%）,C组最低（22.25%）,各组检出率比较,差异有统计学意义（P〈0.05）。A组肠镜插至回盲瓣检查时间（5.3±1.2）min,B组肠镜插至回盲瓣检查时间（5.4±1.1）min,C组肠镜插至回盲瓣检查时间（6.3±1.9）min,三组比较,差异有统计学意义（P〈0.05）,且A组快于B组、C组。结论结肠镜检查前4 h口服西甲硅油乳剂能达到良好的祛泡效果,大大提高结肠镜检查的病灶检出率。

Objective To explore removing foam effect of oral simethicone emulsion in the colonoscopy preoperative.Methods From June 2013 to June 2015, 1200 patients were randomly divided into 3 groups, before colonoscopy 6 hours oral compound polyethylene glycol electrolyte powder. Group A before operation 4 hour oral Spanish silicone oil emulsion,group B before operation 2 hour oral Spanish silicone oil emulsion, group C not taking simethicone emulsion and bubbles, the three groups of cavity volume and lesion detection rate were compared. Results In group A, grade A had355 cases, grade B had 42 cases, grade C had 3 cases. In group B, grade A had 201 cases, grade B had 197 cases,grade C had 2 cases. In group C, grade A had 88 cases, grade B had 121 cases, grade C had 105 cases, grade D had86 cases. The bubble volume in group A was the least, and the group C was the highest, with significant difference（P〈0.05）. The detection rate of lesions in A group was the highest（31.25%）, and the lowest in group C（22.25%）, and the positive rate was significantly different（P〈0.05）. Group A of colonoscopy in to the back of the ileocecal valve inspection time（5.3±1.2） min, group B of colonoscopy in to the back of the ileocecal valve inspection time（5.4±1.1） min,group C colonoscopy in to the back of the ileocecal valve inspection time（6.3±1.9） min, with statistical significance difference among three groups（P〈0.05）. And group A was faster than group B and group C. Conclusion Colonoscopy before 4 hours of oral simethicone emulsion can achieve good bubble removing effect, greatly improve the colonoscopy lesion detection rate.
