头颈部原发性淋巴结外非霍奇金淋巴瘤的CT表现与免疫表型的相关性研究

Correlation of CT and immunohistochemical characteristics of primary extranodal non-hodgkin's lymphoma in otolaryngol head and neck

目的探讨头颈部原发性淋巴结外非霍奇金淋巴瘤的CT表现与细胞免疫学特征的相关性,旨在提高淋巴瘤的诊疗水平。方法收集2010年6月～2015年6月间经病理学确诊的原发于头颈部淋巴结外的非霍奇金淋巴瘤42例,对其CT表现与细胞免疫学特征进行分析。结果 42例患者中T/NK细胞来源者23例（54.8%）,多发生于鼻腔鼻窦,以溃疡型或混合型多见,CT表现为病变形态不规整,边界不清,密度不均匀,增强后轻-中度强化;B细胞来源者19例（45.2%）,多发生于韦氏环,以肿块型多见,CT表现病变形态较规整,边界较清,密度相对均匀,增强后轻-中度强化。两者原发部位、病理类型比较,差异有统计学意义（P〈0.05）。结论不同免疫表型的头颈部原发性淋巴结外非霍奇金淋巴瘤的CT表现有其相对的特征。

Objective To study the correlation of CT characters and cell immunopheno type in cases with primary extranodal non-hodgkin＇s lymphoma（PE-NHL） in otolaryngol head and neck for improving early diagnosis and treatment.Methods The features of CT and cell immunopheno type in forty-two patients with PE-NHL in otolaryngol head and neck were reviewed from June 2010 to June 2015. Results Of 42 cases, 23 patients（54.8%） originatedfrom T/NK cells,most of them arosed nasal cavity, for ulcer or hybrid. The lesions showed irregular morphology, not clear bound, not homogeneous density and slight-mild enhancement 19 patients（45.2%） originated from B cells, while most at waldeyer＇s ring for tumor type ring, the lesions showed regular morphology, clear bound, relative homogeneous density and slightmild enhancement. The differences of primary site and pathological type of two groups were statistically significant（P〈0.05）. Conclusion CT manifestation of different cell immunopheno type PE-NHL are relatively characteristic.
