尿HCMV-DNA检测在新生儿疾病中的应用价值 被引量:9

摘要 目的通过比较不同疾病中的新生儿尿人巨细胞病毒DNA(HCMV-DNA)与血HCMV-IgM抗体的结果,评估尿HCMV-DNA在新生儿疾病中的应用价值。方法采集2013年1月至2014年12月在该院新生儿科住院的1 520例婴幼儿的尿液进行HCMV-DNA荧光定量PCR检测,同时采集其血液采用化学发光方法检测HCMV-IgM抗体。结果 1 520例婴幼儿中尿HCMV-DNA阳性者有153例,阳性率为10.07%,血液标本中的HCMV-IgM阳性者仅有4例,阳性率为0.27%,两者比较差异有统计学意义(P<0.05)。1 520例婴幼儿的疾病构成中,诊断为肝炎综合征的HCMV-DNA阳性率为82.6%,HCMV-IgM阳性率为3.85%。出现黄疸症状的HCMV-DNA阳性率为8.95%,肺部感染的HCMV-DNA阳性率4.62%。各项疾病或症状的HCMV-DNA阳性率比较差异有统计学意义(P<0.05)。结论在诊断婴幼儿HCMV的感染时,尿HCMV-DNA检测比HCMV-IgM抗体更为优越,尤其在肝炎综合征、新生儿黄疸、肺部感染中的应用价值更高。 Objective To evaluate the value of detecting HCMV-DNA in urine in the diseases of newborn babies,by comparing the result between HCMV-DNA in urine and HCMV-IgM antibody in blood in different diseases of the newborn babies.MethodsThe urine and blood samples were collected from 1 520 infants who were in hospital of the neonate department from January 2013 to December 2014.The HCMV-DNA in urine was examined by fluorescence quantitative polymerase chain reaction(FQ-PCR).And HCMV-IgM antibody in blood was examined by the method of chemiluminescence(ECL).Results In the 1 520 cases,153had detectable HCMV-DNA in their urine samples with a positive rate 10.07%,while only 4cases were positive of the HCMV-IgM antibody in blood with a positive rate 0.27%.The two groups was statistically significant difference(P0.05).In the infant diseases of 1 520 cases,the positive rate of HCMV-DNA in urine was 82.6% of the hepatitis syndrome while the positive rate of HCMVIgM antibody in blood was 3.85%.The positive rate of HCMV-DNA in urine was 8.95%in the jaundice symptoms,while the positive rate HCMV-DNA in urine was 4.62%in the pulmonary infection.Each group of disease was statistically significant difference in the positive rate of HCMV-DNA(P0.05).Conclusion The detection of HCMV-DNA in urine is more beneficial to the diagnosis of HCMV infection of infants than the detection of HCMV-IgM antibody in blood,especially in hepatitis syndrome,neonatal jaundice and pulmonary infection.
